Healthcare for the Homeless is looking for a compassionate and self-motivated individual to join our team as Social Worker/Case Manager (SWCM) on a full-time basis. The SWCM will ensure the effective coordination of patient care through collaborative practice within the Health Care for the Homeless (HCH) team. This role provides social work case management, including clinical social work interventions and behavioral health counseling for patients (pediatrics, adolescents, adults, and geriatrics) and their families that address personal, environmental, and socioeconomic issues to maximize emotional, social, and physical well-being and the appropriate and efficient use of healthcare and community resources across the continuum of care.
Responsibilities:
Patient Care
· Complete assessments for mental health, substance use, and social needs in a timely and age-appropriate way.
· Create treatment plans with the team and track progress.
· Join team meetings to discuss care and case management.
· Provide counseling (individual, couples, family), crisis support, and life skills training.
· Lead educational sessions and support relapse prevention.
· Help patients access benefits and services.
· Advocate for patient care and outcomes.
· Support team with care coordination and problem-solving.
· Maintain patient confidentiality.
Quality Management
· Take part in improving care through quality initiatives.
· Help update care policies and identify priorities for improvement.
· Collect and present data related to quality, risk, and patient outcomes.
· Join or lead team meetings focused on reviewing and improving care.
· Offer support for quality tools and track patient progress.
Community Services & Referrals
· Review charts early to identify patient needs.
· Assess family and patient ability to manage care.
· Connect patients to housing, benefits, jobs, and other services.
· Work with community partners for coordinated care.
· Track patient progress and manage referrals.
· Support financial aid applications and provide documentation for insurance.
· Collaborate with hospitals on discharge planning and community placement.
· Master’s Degree in Social Work from a Council on Social Work Education (CSWE) accredited program required.
· 2 years of experience in a Behavioral Health or Outpatient Community Health Care setting.
· Licensed Independent Clinical Social Worker (LICSW) preferred.
· Experience and/or willingness to work with vulnerable populations.
· Valid Driver’s License or the ability to transport self to and from off-site locations.
