About Position:
Role - Hospice CNA/Home Health Aide
Hours - 8:30 -4:30pm, Monday - Friday

Key Responsibilities:
- Provide high-quality, compassionate care to patients in a facility or home-based setting.
- Collaborate with a multidisciplinary team to deliver holistic care.
- Support patients and families with personalized, one-on-one interactions.
- Uphold the values and mission of Catholic Community Health in all interactions.
- Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the primary care nurse immediately.
- Meeting safety needs of the patient and using equipment safely and properly (foot stools, side rails, O2 etc.)
- Assessing and reporting on the patient's condition and significant changes to the assigned supervisory nurse. Also aware of the caregiver or other individuals living with the patient and interpersonal issues.
Requirements
Qualifications:
- Valid CNA license required. HHA license preferred.- We can help getting HHA license
- Strong commitment to providing compassionate, patient-centered care.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Alignment with the values and mission of Catholic Community Health.
Salary Description
Starting at $20 per hour
