As a Geriatric Care Manager, you will play a crucial role in assessing, coordinating, and overseeing the care of seniors, ensuring they receive the highest standard of support and services.

Requirements

Key Responsibilities:

  • Comprehensive Needs Assessment: Conduct thorough evaluations of the physical, emotional, and social needs of elderly clients to create individualized care plans.

  • Personalized Care Planning: Develop tailored care plans that address the unique needs of each client, ensuring their health, safety, and well-being.

  • Care Coordination: Work closely with healthcare providers, home care agencies, and other service providers to arrange essential care and support services.

  • Client Advocacy: Serve as the client’s advocate, ensuring their voice is heard in all matters related to their care, and communicate effectively with family members, medical professionals, and other stakeholders.

  • Crisis Intervention: Provide expert guidance and support in times of medical or emotional crises, ensuring immediate action and care is delivered when necessary.

  • Ongoing Monitoring: Regularly check on clients’ progress and adjust care plans as needed to ensure optimal outcomes.

  • Family Support and Education: Offer guidance and resources to family members, helping them navigate the complexities of eldercare and providing support in decision-making.

Qualifications:

  • Active RN license

  • Certification as a Geriatric Care Manager (CCM or CMC) preferred but not needed.

  • Proven experience in geriatric care management, healthcare, or a related field.

  • Strong communication, organizational, and problem-solving skills.

  • Empathetic, patient, and committed to improving the lives of seniors and their families.
