Description

Job Summary:

Responsible for assessing, planning, organizing, and administering rehabilitative programs that improve mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury.


Essential Functions

  • Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
  • Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
  • Record prognosis, treatment, response, and progress in patient's chart or enter information into computer.
  • Identify and document goals, anticipated progress, and plans for reevaluation.
  • Administer manual exercises, massage, or traction to help relieve pain, increase patient strength, or decrease or prevent deformity or crippling.

Requirements

Job Qualifications:

Education:

Graduation from an accredited physical therapy program.

Licensure:

Current Texas State License in Physical Therapy, current Texas Driver’s License.

Experience:

Two years experience in acute care or rehabilitation setting, home health preferred.

Skills:

Therapy skills as defined as generally accepted standards of practice. Good interpersonal skills.

Transportation:

Reliable transportation and valid and current auto liability insurance.


Environmental and Working Conditions

Works in a patient’s home in various conditions; possible exposure to blood and bodily fluids and infectious diseases; ability to work flexible schedule; ability to travel locally; some exposure to unpleasant weather.


Physical and Mental Efforts:

Requires working under some stressful conditions to meet deadlines and patient needs and to meet patient/family individualized psycho social needs
