Position Summary


The Specialty Care Coordinator is responsible for helping NeighborHealth Center by working collaboratively to support patient care plans and needs. Serving as an advocate for patients, specifically focusing on behavioral health and prenatal care, the Specialty Care Coordinator will be part of the care management team and work in tandem with healthcare providers and community-based organizations to improve outcomes for patients they serve. Working within his/her scope of practice, this role coordinates between healthcare services, recognizing the holistic needs of the patient, inclusive of patient-specific social and cultural dynamics. 


Principal Duties and Responsibilities*

For Behavioral Health:

• Provide coordination for BH patient appointments, including scheduling, confirmations, and addressing no-show/canceled appointments.

• Follow up on patient inquiries as designated by the BH providers.

• Support internal BH provider scheduling, availability, and referrals, including closing the loop.

• Provide coordination of care for all external behavioral health referrals, including sending, following up, closing the loop, and uploading appropriate documentation into the EMR. 

• Maintain BH referral resource information and documentation for NHC providers, including details on availability and specialties, while also building relationships with community resources/providers. 

• Organize and maintain BH resources and ensure exam rooms are stocked appropriately with BH resources. Develop new resources as needed/requested.

• Assist with crisis management as a part of the behavioral health team as needed.

For Prenatal Care:

• Assist with the coordination of care for prenatal patients, including maintaining the registry of pregnant women, participating in care team meetings, and following up on care coordination tasks as required.

• Track all prenatal patients for which NHC provides care, ensuring appointment scheduling and follow-up, postpartum care, coordination with hospitals (including scheduling induction of labor), maternal SDOH & BH screenings are completed, and resources are provided.

For all Care Coordination:

• Support NHC general care coordination duties as required.

• Provide education to patients/family within the scope of practice.

• Serve as a liaison among the patient/family, community services, primary providers, specialists, and other care team members to coordinate services.

• Maintain appropriate documentation in the EMR as necessary.

• Partner with Quality Improvement director to improve efficiency and effectiveness of patient health outcomes.

• Provide backup support for NHC referral coordinator(s)

• Abide by Health Center guidelines, policies, and procedures, and HIPAA regulations.

• Attend departmental and corporate meetings, local and regional training, or other events as required.

• Willingly perform other duties as assigned.

• Provide educational information to the care team, patients, family, and caregivers about community-based organizations (existing and new) within the service area.


Requirements

Required Skills or Abilities*

• Ability to stand, sit, talk, hear, reach, stoop, and kneel.

• Ability to tolerate and work with moderate ambient noise and sometimes in a confined area.

• Ability to work at a computer for extended periods.

• Current NC driver's license and auto insurance


Required Knowledge, Experience, or Licensure/Registration

• High School education or equivalent.

• Ability to cultivate and develop inclusive and equitable working relationships with co-workers and community members.

• Ability to serve as an advocate for individuals of all ethnicities, genders, ages, and backgrounds.

• Ability to work a schedule Mondays through Fridays during the hours of 8 am to 5 pm with hours outside this time frame when needed; may work remotely when approved by supervisor and would require a home office with secure Internet access.

• Ability to read, write, speak, and comprehend English fluently and communicate clearly and succinctly.

• Possess excellent verbal and written communication skills.

• Must be able to work with changing priorities.

• Requires excellent organizational, critical thinking, and problem-solving skills.

• Must be able to work with individuals in all levels of authority.

• Must be able to maintain confidentiality.

• Must function well in a team environment.

• Ability to use a computer in a Windows environment and proficient in MS Word, Outlook, Excel, Internet tools with keyboarding skills of 45 wpm.

• Vaccines as required.


This is a full-time, nonexempt position.
