Position Title:  SENIOR SERVICES COORDINATOR

Responsible To: SENIOR ADMINSTRATIVE OFFICER

Exposure Level: CATEGORY 1


Summary:

This multifaceted position requires a skilled individual to coordinate and deliver various services to seniors through innovative home and community-based services. The role encompasses service coordination, client management, community engagement, and leadership responsibilities.  


Internal Relationships: Weekly contact with Members, staff Members, volunteers and member’s family in completing their assigned job functions.


External Relationships: Contact with other community businesses, agencies to coordinate care for Members as need, donors and, community members to further the mission of St. John’s United. 


ESSENTIAL JOB FUNCTIONS: Service Coordination

· Assist with development of Five Valleys @At Home in partnership with St. John’s Senior Administrative Officer.

· Conduct senior and family caregiver needs assessments as a part of a Rural Aging Action Network (RAAN) Grant. 

· Promote coordinated support for family caregivers who care for family members who struggle with dementia-related diagnoses through St. John’s United’s GUIDE Partnership. 

· Support the President of Five Valleys Senior Care and Foundation in advancing the organization's charitable work and raising funds. 

· Engage with community organizations and leaders. 

· Develop and distribute promotional materials. 

· Develop project delivery and tracking procedures, and follow-up procedures. 

· Coordinate RAAN Grant meetings and action items. 

· Conduct regular check-ins with program staff. 

· Participate in GUIDE meetings and training. 

· Create or identify an assessment tool for caregiver and client needs. 

· Engage in community activities. 

· Provide in-home respite care as needed. 

· Perform duties in clients' homes to support their independence. 

· Travel throughout the community


ESSENTIAL JOB FUNCTION: Concierge

· Coordinate and ensure essential services and benefits, arranging personal care, housekeeping, meal prep, transportation, and emergency services. 

· Implement and communicate a monthly schedule of activities. 

· Available After Hours/On Call

· Disseminate project information and connect clients to resources. 

· Prioritize key volunteerism and senior network gatherings. 

· Field calls and conduct in-person client assessments. 

· Manage client wait list and perform client-volunteer matches. 

· Provide temporary relief to primary caregivers of older adults. 


ESSENTIAL JOB FUNCTION: Communication/Reporting

· Communicate with program participants, RAAN staff, community partners, and others. 

· Speak at community gatherings, civic organizations, and churches. 

· Network with referral sources for clients and volunteers. 

· Maintain contact with clients, families, case managers, and partner sites. 

· Maintain a CRM database for management and reporting. 

· Communication through use of technology helpful.

ESSENTIAL JOB FUNCTION: Employee Responsibilities

· Follows policy and procedures on initial TB testing.

· Follows policy and procedures on annual influenza vaccinations.

· Attends all mandatory in-services.

· Attends and participates in scheduled training and education classes to maintain current certification in accord with the State of Montana.

Requirements

  

REQUIREMENTS: 

· High school diploma or GED required, accredited degree or equivalent experience in relevant fields preferred. C.N.A. Certified or ability to complete SJU CNA Course within 6 months of hire. CPR certification preferred or must obtain within 6 months. Valid Montana Driver’s License with clean record.


Knowledge, Skills/Abilities: 

· Be knowledgeable regarding various types of fund development campaigns, the fund development process, and Planned Giving.

· Possess excellent written and verbal communication skills. Previous experience with grant proposals and working with the media preferred. 

· Be self-directed, highly responsive and possess strong inter-personal skills.

· Ability to work with diverse individuals and families compassionately. 

· Ability to lift and transfer clients. 

· Training in computer skills, data management, and customer service. 

· Organizational and problem-solving skills; ability to work independently and in teams. 

· Knowledge of community resources preferred. 

· Experience in program development, research, and team management preferred.  

· Knowledge and skills acquired through general life experience

· Proficient at housekeeping, basic maintenance, and knows their way around the kitchen.

· Have general knowledge of technology.


Physical Demands

  • Requires heavy physical work more than 30% of the time, including      lifting up to 25 pounds regularly and up to 50 pounds occasionally. Ability to stand, climb a      ladder/comfortable with heights, push, pull, lift, walk, and bend for      prolonged periods of time. Must be      comfortable driving a company owned vehicle.
