- Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.) from an accredited medical school
- Completion of accredited Residency Program
- Board-certified or board-eligible in pediatrics
- License or eligible for licensure in Indiana
- Active and unrestricted DEA license or ability to obtain prior to start
- Prefer working at an FQHC, CHC, or PCMH
