Job Title: MDS Coordinator - Long Term Care
Job Summary:
We are seeking a dedicated and detail-oriented experienced MDS Coordinator who is looking at a regional roll to join our corporate-team. The ideal candidate will be responsible for overseeing several facilities in the completion of the Minimum Data Set (MDS) assessments and ensuring compliance with federal and state regulations. This role is crucial in enhancing the quality of care for our residents and supporting the interdisciplinary team while training and educating the facility MDS coordinators.
Key Responsibilities:
- Conduct and coordinate MDS assessments multiple facilities for residents in accordance with regulatory requirements.
- Collaborate with nursing staff and other healthcare professionals to gather necessary information for accurate assessments.
- Ensure facilities are timely submitting MDS documentation to appropriate agencies.
- Monitor and analyze resident data to identify trends and areas for improvement in care plans.
- Provide training and support to staff on MDS processes and documentation standards.
- Stay updated on changes in regulations and best practices related to MDS and long-term care.
Skills and Qualifications:
- Degree in nursing, healthcare administration, or a related field preferred.
- Valid nursing license or certification in long-term care.
- Strong knowledge of MDS processes and regulations.
- Excellent organizational and time management skills.
- Strong communication and interpersonal skills.
- Ability to work collaboratively in a team-oriented environment.
- Proficient in using electronic health record (EHR) systems and MDS software.
