Location: Community Health Center of the North Country - Ogdensburg, NY
Full-Time | Monday–Friday, Daytime Hours | No Nights, Weekends, or Holidays
Pay Range: $18.72 - $25.00 per hour

We’re seeking a skilled and dedicated Licensed Practical Nurse (LPN) to join our collaborative care team in a welcoming outpatient setting.
What You’ll Do
- Greet and prepare pediatric and adult patients for visits, including taking vitals and updating medical histories
- Prepare charts and review records to ensure timely and organized visits
- Assist providers during exams, procedures, and treatments
- Accurately document patient information in the electronic medical record (EMR)
- Respond to routine patient phone messages, escalating concerns to the RN or provider as needed
- Process prescription refills and coordinate with pharmacies
- Monitor and manage clinic supplies and vendor equipment deliveries
- Support infection control protocols and help maintain a safe, welcoming care environment
- Participate actively in team meetings and quality improvement efforts
What You’ll Need
- Current New York State LPN license
- Current BLS certification (training can be provided post-hire)
- Strong communication, clinical, and organizational skills
- Experience with electronic medical records preferred
- Commitment to professionalism, teamwork, and confidentiality
