Provides ongoing support for participants, ensures continuity of care and referrals, and tracks long-term outcomes for workforce transition success.
Key Responsibilities:
• Maintain regular contact with participants post-placement
• Offer ongoing barrier reduction support and coaching
• Coordinate re-engagement or upskilling referrals as needed
• Track retention metrics and long-term outcomes
• Provide follow-up reporting for compliance
