- Document SDMI Waiver referral information and coordinate with the regions Clinical Lead.
- Receive communications via phone, and email regarding waitlist information
- Contact individuals on the SDMI Waiver waitlist quarterly and update all necessary information.
- Update Level of Impairment form for individuals on a yearly basis.
- Follows State of Montana and A.W.A.R.E., Inc. rules, policies, procedures, and guidelines as they apply to this service.
- Assures that any person serviced by Adult Case Management is screened for and assisted in enrollment in the SDMI Program.
- Must demonstrate – on a consistent basis – the willingness and ability to productively utilize work time in order to meet the needs of the individuals being served as directed, with the understanding that specific fiscal goals (billing, case load number maintenance, weekly) must be met on a monthly basis.
- Record referrals, and other data entry assignments via AWARE’s Electronic Healthcare Record System.
Talents, skills, and abilities:
- AWARE SDMI referral coordinators must have the interpersonal oral and written communications skills to work as a team member, and to follow oral and written instructions.
- A AS/AA Degree and minimum of one (1) year of direct work experience is preferred. An education equivalency can be established with combination of education and pertinent employment experience.
- A valid Montana Driver’s License or the ability to obtain one, and an acceptable driving record.
- An acceptable criminal and protective services background check.
