Provides hands-on care to facility residents while monitoring and supporting their health and wellbeing. Manages and administers medications, completes assessments, and coordinates with other healthcare professionals as needed. Provides support and education to residents and their families.

Responsibilities:

• Quarterly assessment to evaluate physical and mental health of residents
• Coordinate with providers, therapists, and other specialists to ensure residents receive appropriate care
• Administer medications and monitor for effectiveness
• Observe and report changes in health status to appropriate parties
• Assist residents with activities of daily living
• Document assessments, medications, and changes in health status
• Provide emotional support to residents and foster a sense of security

• Current North Dakota RN or LPN license
• BLS certification
• Experience with geriatric population is preferred
• Ability to communicate effectively in English
