POSITION SUMMARY: This position provides employment services to consumers, including assessment of consumers’ strengths and needs utilizing person centered principles, job development, placement, coaching, extended support services and collaboration with other service providers and family members and/or life supports, as allowed. It includes meeting productivity measures and daily documentation requirements.

EDUCATION & EXPERIENCE REQUIREMENTS: Prefer B.S. in Vocational Rehabilitation, Psychology, Social Work or related field and minimum of two years positively referenced relevant work experience.
SCHEDULE: Flexible based on consumer needs
- Mental health and/or psychiatric rehabilitation experience and/or knowledge sufficient to understand consumer needs.
- Interpersonal skills sufficient to establish productive working relationships.
- Read, write, and speak English well enough to communicate with others and maintain documentation and perform other record keeping tasks in accordance with agency and accreditation standards.
- Computer skills sufficient to enter the required data and documentation.
- Pass the agency’s background check.
- Clean driving record to support insurability by the agency’s insurance carrier.
- Valid driver’s license and ability to drive legally and safely transport consumers in own vehicle if needed.
- Work related use of a vehicle carrying minimum of 100/300/50 liability insurance.
- Possession of CPR/First Aid certification.
ADDITIONAL DUTIES & RESPONSIBILITIES – SPECIFIC TO POSITION
- Assist assigned individuals in assessing their strengths, needs, resources and desires as related to their vocational goals. Complete Vocational Assessment annually, or as needed, utilizing person-centered principles.
- Assist assigned individuals in developing an Individual Vocational Plan (IVP) within 30 days of entry and every 6 months at a minimum.
- Complete DORS reports, including Progress Reports, as needed.
- Support consumers in meeting their vocational goals.
- Plan and direct structured activities focused on vocational skill development.
- Assist consumers with understanding the relationship between employment and the benefits and entitlements they are receiving, if any, and in preparing for appointments with benefits counselors.
- Maintain accurate, timely and professional documentation in accordance with all Agency policies and applicable regulations, including but not limited to: daily service documentation, contact notes, ACT/STATs, Progress Notes, Incident Reports, travel reimbursement forms and time cards.
- Maintain a flexible schedule to meet the needs of the consumer.
- Provide support services and encouragement on the job, in the community, or in the consumer’s home, as needed.
- Advocate for the services and systems that best address consumers’ identified needs.
