The Community Engagement Navigator supports Frontera’s mission by assisting with outreach and engagement efforts across the service area. This position helps strengthen community partnerships, participates in outreach activities, and promotes access to healthcare services, particularly for Medicaid and Medicare populations. The Navigator provides direct support for community events, health fairs, and patient engagement initiatives while representing Frontera in a professional and approachable manner. This role works closely with the Community Engagement Navigation Manager and other team members to ensure consistent messaging, quality outreach, and alignment with organizational goals.
We are seeking a dynamic and passionate Community Engagement Navigator to serve as a vital bridge between our organization and the communities we serve. This individual will focus on building meaningful relationships with community members, partners, and local organizations while also supporting the marketing team to ensure that community voices and stories are authentically reflected in outreach and promotional efforts.
This is a collaborative, cross-functional role that requires strong interpersonal, organizational, and communication skills, as well as creativity and an understanding of how community engagement supports both mission-driven work and strategic marketing goals. Must be willing to attend community events after hours and weekends.
Key Responsibilities
Community Engagement
• Represent Frontera at community events, health fairs, school programs, and outreach activities.
• Support building relationships with local organizations, schools, businesses, and community stakeholders.
• Assist in organizing and participating in Community Stakeholder Team meetings and activities.
• Provide information to community members about Frontera’s services and programs.
Outreach & Enrollment Support
• Obtain and maintain Community Partner Program, Community Health Worker, Health Insurance Marketplace and any other certifications as directed.
• Assist patients and families with referrals to enrollment, navigation, and available community resources.
• Help promote Frontera’s services with a focus on reaching underserved populations.
Qualifications and Skills:
Required:
• High school diploma or equivalent.
• Experience in community outreach, health education, customer service, or related work.
• Strong verbal and written communication skills with the ability to engage diverse community members.
• Ability to work independently and as part of a team.
• Commitment to serving underserved communities and advancing health equity.
• Bilingual in English and Spanish.
Preferred:
• Associate’s or Bachelor’s degree in Public Health, Communications, or related field.
• Familiarity with Medicaid and Medicare populations.
• Experience working in a healthcare or nonprofit setting.
Special Requirements:
After hours, weekends and travel is required. The employee must be able to lift and/or move more than 50 pounds. Requires sitting, standing and walking for extensive periods of time. Requires working under stressful conditions or working irregular hours. Ability to work with a moderate noise level in the work environment is required.
