Reporting to the Paraprofessional Administrative Manager, the Granite VNA Home Health & Hospice Aide performs personal care and designated health-related services for the care and well-being of assigned clients under the supervision of a Registered Nurse. The Home Health & Hospice Aide interacts with client’s family members, facility staff and other healthcare team members while maintaining professional standards.


The Granite VNA Home Health and Hospice Licensed Nursing Assistant (LNA) performs personal care and designated health-related services for Home Health and Hospice patients in their homes. The Home Health and Hospice LNA interacts with client’s family members and other healthcare team members.


Essential Job Functions

  • Follows care plan in providing basic care and services to the resident including assistance with Activities of Daily Living (ADL): walking, bathing, shaving, mouth care, grooming, eating transfers, and management of incontinence.
  • Communicates with nurses involved in the resident’s care plan; observes, records and reports changes in resident’s condition, status, or other pertinent information to the nursing staff.
  • Does laundry, housekeeping and maintenance to assure a safe, orderly, clean and comfortable environment.
  • Provides supervision to patient/residents with cognitive deficit.
  • Ensures residents have the opportunity to engage in cultural, spiritual, physical, political, social and intelligent activities as the residents’ desire.
  • Maintains calm, rational manner when dealing with others, even in situations involving conflict or crisis.
  • Documents care provided to the patient/resident accurately in accordance to the agency’s guidelines.
  • Works independently on individual assignments yet support the needs of the total work unit.
  • Complies with the Agency’s documentation, time entry scheduling requirements
  • Communicates clearly and courteously with others including client, family members, and other agency staff.
  • Practicing safe work habits.
  • Attending Agency meetings and in-service programs.

Other Job Functions

Provides assistance and support to volunteers
Respects volunteers as an integral part of the Hospice Team
Performs other duties as assigned.

Full time position Monday - Friday 8:00 a.m. - 4:00 p.m. No weekend or Holiday.

Minimum Qualifications and Requirements
High school diploma/GED, accompanied by current New Hampshire LNA license
Reliable automobile
Automobile Insurance
Valid Driver’s license
