Seniors At Home offers a wide range of services to help older adults live safe, healthy, and independent lives, including home care, personal assistant services, companionship, and specialized dementia and palliative care. We provide the Bay Area’s leading continuum of care for aging adults.


POSITION SUMMARY:

Under the supervision of the Director of Home Care, the Home Care Supervisor is responsible for managing home care services for Holocaust Survivor clients and coordinating the initiation of client services in collaboration with the Director of Holocaust Survivor Programs and assigned Care Manager. Key responsibilities include ensuring that ADL care plan needs are met, working with staffing coordinators to ensure all client hours are appropriately staffed, and monitoring that home care aides to ensure all training and continuing education requirements are in compliance with CDSS regulations.


COMPENSATION AND BENEFITS:

  • Pay Range: $34.87 - $36.92 (depending on experience)
  • Employment Type: full-time, non-exempt position with benefits

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Upon completion of all initial paperwork, initiates the client onboarding process
  • Explains all home care service policies to the client, including the role of the Claims Conference, SAH responsibilities in providing the allotted hours of care, and reviews and completes all onboarding paperwork
  • In conjunction with the client, family, and home care aide, the HCS completes an individualized care plan at the client’s home
  • Conducts a safety assessment during the initial consultation with the client, home care aide, and/or family member
  • Collaborates with the Care Manager to implement the client care plan through home care visits, as directed by the Director of Holocaust Survivor Programs and/or Care Manager
  • Conducts in-home supervision per established protocols and assists in the evaluation and supervision of caregivers
  • Communicates all changes in client condition to the Care Manager or Director of Holocaust Survivor Programs promptly, so the DAF is updated in a timely manner
  • Supervises and assists in resolving matters between home care aides and families
  • Provides new client follow-up per department policy, adhering to the new start-of-care follow-up process, including addressing clients’ questions and concerns to ensure satisfaction
  • Participates in the after-hours on-call rotation to support staffing coordinators as needed, and provides management support to clients and home care aides


JOB QUALIFICATIONS:

  • Russian language proficiency is required
  • Minimum of three years of professional experience in Home Care or Social Services setting is required
  • Strong commitment to providing responsive services to the community and to establishing excellent rapport with clients, staff and community workers
  • Demonstrated ability to convey complex information clearly to clients, home care aides and leadership
  • Ability to develop trust and credibility by following through on tasks and commitments
  • Demonstrated ability to build strong team relationships and foster a supportive work environment
  • Demonstrated skills in the use of Microsoft Word, Excel, Access, Outlooks, and other computer software
  • Ability to address client and home care aide concerns with sound judgment and professionalism
