HOSPICE LPN

Position Summary:
Support the Registered Nurse and interdisciplinary team in implementing the hospice plan of care. Provide compassionate, palliative nursing care, document services accurately, and communicate effectively with team members. Reports to the Hospice Manager.

Key Responsibilities:

  • Deliver nursing care in accordance with organizational policies, procedures, and within LPN scope of practice.
  • Assist the RN and physician with specialized tasks related to end-of-life care.
  • Contribute to the implementation of the hospice plan of care under RN supervision.
  • Prepare clinical and progress notes to document patient care and intervention outcomes.
  • Set up and manage equipment and materials using proper aseptic techniques.
  • Support patients in learning appropriate self-care techniques.
  • Monitor and report patient/family responses to care.
  • Ensure timely communication with interdisciplinary team members.
  • Perform other duties as assigned.



Requirements

Unencumbered state license as a Licensed Practical Nurse

One year of nursing experience, preferably in hospice, home health, or skilled nursing

Current Basic Life Support (CPR) certification

Proficient in English (spoken and written)

Strong verbal and written communication skills

Ability to meet deadlines and work independently

Able to perform CPR and airway obstruction relief

Physically able to walk or stand for at least 4 hours and climb two flights of stairs

Valid driver’s license with a satisfactory driving record

Proof of current auto insurance

Must complete 12 hours of continuing education, annually
