Positive Impact Health Centers (PIHC) is a community leader in providing HIV prevention, care and treatment services. The PIHC model of care assures that persons with HIV have access to medical, pharmacy, dental, behavioral health and social services, providing the best opportunity for patients to achieve high-quality health outcomes.


 Essential Functions:

  • Provide "Red Carpet" experience for clients referred from community CBOs to relevant HIV care and social service providers. 
  • Accompany clients to service delivery sites to ensure successful engagement and linkage to care. 
  • Support clients in accessing HIV care, including primary care, mental health, substance use, housing, and other supportive services. 
  • Expedite linkage to HIV care and treatment services by coordinating appointments and transportation as needed. 
  • Ensure that clients are connected to the appropriate services and remain engaged throughout the care continuum. 
  • Collaborate with CBO leadership and the Project Manager to ensure streamlined communication and smooth transitions for clients from CBOs to service providers. 
  • Assist in coordinating client care between community-based organizations, healthcare providers, and other relevant services.

PREFERRED KNOWLEDGE, SKILLS, ABILITIES & OTHER APTITUDES (KSAOs):

  • Strong understanding of HIV care, prevention, and treatment services.
  • Experience working with marginalized communities, particularly racial and ethnic minorities and LGBTQ+ populations.
  • Excellent communication and interpersonal skills.
  • Ability to provide compassionate, client-centered support.
  • Knowledge of local HIV service networks and social service providers.
  • Organizational and time-management skills to balance multiple tasks and priorities.


Requirements


  • High school diploma or GED required; Bachelor’s degree in Social Work, Public Health, or related field preferred. 
  • Minimum of 2 years of experience in community outreach, case management, or client navigation services, particularly in HIV care. 
  • Valid State Driver’s License 
  • Must be able to travel locally: between Positive Impact Health Centers and community-based organizations is required. 
  • Occasional travel to attend training or community events as needed.
