Description
The RN Case Manager is responsible for coordinating and managing the care of patients across various healthcare settings, ensuring the implementation of individualized care plans and providing high-quality nursing care. This role requires strong clinical knowledge, excellent communication skills, and the ability to collaborate effectively with an interdisciplinary healthcare team.
Schedule: Full-Time Monday through Friday 8AM-5PM
Primary Responsibilities and Duties:
- Conduct comprehensive patient assessments to identify healthcare needs, develop individualized care plans, and update plans as needed.
- Provide skilled nursing care, including wound care, medication administration, and patient education, in accordance with the care plan and physician orders.
- Coordinate and supervise the activities of healthcare professionals involved in patient care, such as nursing staff and therapists.
- Communicate effectively with patients, families, physicians, and other members of the healthcare team to update them on patient progress, discuss treatment plans, and address any concerns.
- Monitor patients' progress, evaluate the effectiveness of care plans, and make adjustments as necessary.
- Ensure compliance with regulatory guidelines, documentation requirements, and facility policies and procedures.
- Collaborate with an interdisciplinary healthcare team to develop and implement patient care strategies, considering the patient's physical, emotional, and social needs.
- Participate in team meetings to discuss patient care plans, progress, and discharge planning as needed.
- Perform all other duties assigned by the Director of Nursing or other designated supervisor.
Requirements
Job Qualifications:
- Valid Registered Nurse License (RN) in the state where work is performed.
- 1 or more years of experience as a case manager or related role in a healthcare setting.
- Strong assessment and critical thinking skills, with the ability to make sound clinical judgments.
- Excellent interpersonal and communication skills, with the ability to collaborate effectively with patients, families, and team members.
- Commitment to providing high-quality, patient-centered care with empathy and compassion.
- Proficient in basic computer software programs and electronic medical record systems.
- Knowledge of regulatory requirements and guidelines governing case management and documentation in healthcare settings.
- Strong organizational and time-management skills, with the ability to handle multiple tasks and prioritize effectively.
Physical Requirements:
- Ability to lift and transfer patients, as well as carry equipment and supplies weighing up to 50 pounds.
- Capable of prolonged standing, bending, squatting, and walking during home visits.
- Fine motor skills for documentation and administering treatments.
- Ability to respond swiftly and efficiently to emergency situations.
- Maintain regular attendance and timeliness as set forth within the work schedule designated by the department supervisor/manager.
- Job requires accepting criticism and dealing calmly and effectively with high-stress situations.
- The ability to use your abdominal and lower back muscles to support part of the body repeatedly or continuously over time without "giving out" or fatiguing.
Salary Description
$40 - $42 Per Hour
