Occupational Therapist - OT Location: Leo-Cedarville & Fort Wayne, Indiana
Schedule: Full-time I 30-40 hours per week I Monday through Friday

We are seeking a full-time Occupational Therapist to join our therapy teams at two well-regarded communities in Allen County. This role offers both variety and continuity, serving diverse populations across multiple care settings. Paid drive time and mileage reimbursement are provided for travel between the two sites (approximately 15 miles apart). Schedule may include three days at primary site and two days at the secondary site, or another balanced arrangement.


Site Information: 


Leo-Cedarville (Primary Site) A welcoming continuing care retirement community offering skilled nursing, assisted living, and independent living, with memory care coming soon. The therapy gym is well equipped and includes a training kitchen and bathroom to support functional, real-life interventions.


Fort Wayne (Secondary Site) A progressive, wellness-focused community located near Parkview Hospital, serving adults with complex medical, cognitive, and functional needs. Care is grounded in dignity, purpose, and person-centered support, with strong interdisciplinary collaboration. The community offers a spacious therapy gym, varied equipment, and treatment spaces designed to support meaningful, real-world outcomes.

Requirements
  • Active Indiana Occupational Therapist license or must have the necessary qualifications to earn Indiana license.
  • Degree from an ACOTE-accredited occupational therapy program
  • Successful completion of the National Board of Certification in Occupational Therapy (NBCOT®) examination
