Description

Exciting opportunity to join a fast-paced medical team within a growing federally qualified community health center. HealthFirst Family Care Center, Inc. “The Heart of Our Community” provides comprehensive services including adult/pediatric medical care, health education, health assessment and screenings, gynecology, cardiology, behavioral health, substance use treatment, and adult/pediatric dental care. 


RN Care Manager (C3) Summary:

As an integral member of the care management team, the RN Care Manager (RN CM) will have the opportunity to have a profound impact on the lives of people living with complex and/or chronic conditions, many of whom also face multiple barriers in their lives, which make it difficult for them to achieve the self-care required to improve their health and well-being. This position is currently hybrid, but requires flexibility, and may vary from day-to-day to meet members where they are. Outreach methods are based on the needs of the organization and the member, and may include telephonic, or in-person engagements in a variety of potential settings, such as the health center/practice, community, home, or an inpatient facility. The RN CM is under the general supervision of the Director of Nursing.


Schedule:

Mon – Fri: 8:30 am – 5:00 pm


Pay Range:

$81,300 - $88,000 annually


Required Qualifications:

  1. Experience within the ACO’s member population preferred, including Medicare/Medicaid member populations.
  2. Experience working with Federally Qualified Health Centers/ Primary Care Provider practices is strongly preferred.
  3. Associate’s degree in nursing is required. Bachelor’s degree in nursing is a strong plus.
  4. License to practice as an RN from the Commonwealth of Massachusetts required.
  5. 3-5 years of nursing experience; home health care, ambulatory care, community public health or Case/Care Management.
  6. A valid driver's license and provision of a working vehicle.
  7. Experience working with patients with chronic medical and behavioral health needs preferred.
  8. Maintain current certification in CPR at the healthcare provider level.
  9. Bi-lingual in Spanish or Portuguese preferred.
  10. Must submit a CORI Release Form and demonstrate proof of being free from disqualifying information.

  5. Must be flexible and adaptable to change.
  6. Demonstrate the ability to work independently.
