We are looking for a compassionate and dedicated Nurse who believes that quality health care starts with dignity, advocacy, and human connection. In this role, you’ll be a trusted clinical leader—supporting individuals with developmental and intellectual disabilities, their families, agency staff and care teams while ensuring health and safety needs are met with respect, empathy, and excellence

What a Day to Day would look like for you/ Job Duties
- Advocate for individuals receiving services, ensuring their health, safety, and dignity are always at the center of care.
- Coordinate and oversee comprehensive health care needs, including assessments, care planning, follow-up, and documentation.
- Create and maintain individualized health care plans and provide thoughtful training to employees and contractors who support each individual.
- Mentor and support care staff through guidance, education, and encouragement—helping them grow in confidence and competence.
- Monitor health progress and collaborate with interdisciplinary teams to ensure continuity and quality of care.
- Oversee medication management across the agency, including MAR reviews, pharmacy coordination, and audits.
- Track medical appointments, review provider recommendations, and ensure follow-through on all care needs.
- Respond to health concerns, illnesses, or injuries with calm, sound nursing judgment and hands-on support when needed.
- Review incident and health reports promptly, ensuring individuals receive timely and appropriate care.
- Provide emotional support and clear communication to individuals and families during times of change or concern.
- Utilize Therap and other systems in accordance with state and agency standards.
- Participate in quality improvement efforts, audits, and compliance activities.
- Provide after-hours and weekend on-call nursing support as needed.
- Work closely with leadership, service coordinators, and program teams to support a culture of compassion and accountability.
What You Bring
- A current nursing license and knowledge of the NM Nurse Practice Act
- A deep commitment to person-centered, compassionate care

- Confidence training, mentoring, and supporting care staff
