Schedule: 

Monday-Friday 8am-4:30pm


SUMMARY 

The Outreach/Vocational Case Manager focuses on identifying, engaging, and securing housing for eligible individuals, while also supporting participants in achieving self-determined vocational and employment goals. This role bridges outreach, housing placement, and employment support by engaging individuals where they are, facilitating access to housing, and coordinating community-based vocational and employment services that promote long-term stability and independence.


ESSENTIAL DUTIES AND RESPONSIBILITIES  

  • Identify and locate eligible individuals in need of housing by conducting outreach.
  • Attend/conduct meetings providing information about homeless programs to other providers.
  • Conduct surveys and complete homeless assessments for potential participants and enter this information into Homeless Management Information System (HMIS).
  • Collaborate with the Coordinated Entry staff regarding the Coordinated Entry list.
  • Participate in committees through the Points North Housing Coalition (PNHC) Continuum of Care (CoC).
  • Assess clients for vocational and employment needs.
  • Develop vocational and employment goals and objectives with each client.
  • Coordinate services necessary for attainment of goals.
  • Provide care coordination services and complete referrals to additional services to meet client needs.
  • Monitor and document clients’ progress toward employment goal attainment and post-employment maintenance.
  • Collaborate with the Assistant Director of Housing, Sr Case Manager and other case managers to ensure continuity of services.
  • Report all pertinent information concerning illnesses, accidents, incidents, etc. to supervisor.
  • Maintain required documentation according to agency guidelines.
  • Develop and maintain professional community relationships to promote services.
  • Perform problem-solving functions to assist individuals in overcoming obstacles preventing obtainment of housing.
  • Monitor and maintain high standards of service delivery.
  • Maintain client confidentiality at all times, including 42 CFR/HIPAA.
  • Follow agency policies and procedures.
  • Attend and participate in relevant trainings.
  • Attend staff meetings, supervisions and conferences as assigned.
  • Performs other duties as assigned or that may develop.

QUALIFICATIONS

EDUCATION and/or EXPERIENCE 

Required: Associate’s Degree in the Human Services field and two years of experience in the Human Services field, preferably with individuals with mental health diagnoses and/or substance use disorders. 


Preferred: Bachelor’s Degree in Human Services and 1 year of Human Services experience, preferably with individuals with mental health diagnoses, substance use or co-occurring disorders.


CERTIFICATES, LICENSES, REGISTRATIONS

Required: Must maintain valid driver’s license and acceptable driving record.

$22/hr
