Heartline Staffing, in partnership with Purple Cow Recruiting, is seeking an experienced Travel Registered Nurse – Case Manager for an acute care hospital assignment in Mission Hills, California.

This role supports inpatient care coordination and discharge planning for medically complex patients in a structured hospital environment with strong interdisciplinary collaboration.

POSITION OVERVIEW
The RN Case Manager is responsible for coordinating patient care across the continuum, facilitating safe and timely discharges, and ensuring compliance with payer and regulatory requirements.

ASSIGNMENT DETAILS
• 13-week contract
• Day shift | 5x8s (08:00–16:30)
• Weekend rotation required
• Patient ratio: approximately 1:20
• Acute care hospital setting
• Epic EMR
• No floating required

COMPENSATION
• Gross: $2,752 per week
• W2: $24.00 per hour
• Tax-Free Stipend: $1,792 per week

CORE RESPONSIBILITIES
• Coordinate inpatient care and discharge planning
• Collaborate with physicians, social services, and external providers
• Manage patient throughput and length of stay
• Apply utilization review and payer guidelines
• Arrange DME, SNF placement, and transportation
• Document accurately in Epic EMR

WORK ENVIRONMENT
• Acute care hospital setting
• Adult and geriatric patient population
• High-volume case management workflow
• Scrubs or business attire permitted

• Minimum 2 years acute care Hospital Case Manager experience REQUIRED (HARD STOP)
• MUST have inpatient care coordination and discharge planning experience
• InterQual experience STRONGLY PREFERRED
• Epic EMR experience REQUIRED
• Active California RN license REQUIRED (pending accepted, must be active prior to start)
• Ability to manage ~1:20 caseload REQUIRED
• Weekend rotation required

• Behavioral health-only case management NOT accepted
• First-time travelers accepted with clinical screening
• RTO limited to 7 days
