Heartline Staffing, in partnership with Purple Cow Recruiting, is seeking an experienced Travel Registered Nurse – Case Manager for an acute care hospital assignment in Mission Hills, California.

This role supports inpatient care coordination and discharge planning for medically complex patients in a fast-paced hospital environment.

POSITION OVERVIEW
The RN Case Manager is responsible for coordinating patient care across the continuum, ensuring safe and timely discharges, and collaborating with interdisciplinary teams including physicians, social services, and external providers.

ASSIGNMENT DETAILS
• 13-week contract
• Day shift | 5x8s (08:00–16:30)
• Weekend rotation required
• Patient ratio: approximately 1:20
• Acute care hospital setting
• Epic EMR
• No floating required

COMPENSATION
• Gross: $2,752 per week
• W2: $24.00 per hour
• Tax-Free Stipend: $1,792 per week

CORE RESPONSIBILITIES
• Coordinate inpatient care and discharge planning
• Perform utilization review using InterQual criteria
• Collaborate with interdisciplinary teams and external providers
• Arrange DME, SNF placement, and transportation services
• Ensure compliance with payer and regulatory guidelines
• Document accurately in Epic EMR

WORK ENVIRONMENT
• Acute care hospital setting
• Adult and geriatric patient population
• High-volume care coordination workflow
• Business casual or scrubs permitted

• Minimum 2 years acute care Hospital Case Manager experience REQUIRED (HARD STOP)
• Experience with Care Coordination and Discharge Planning REQUIRED
• InterQual criteria experience REQUIRED
• Epic EMR experience REQUIRED
• Travel experience REQUIRED
• Active California RN license REQUIRED (pending accepted, must be active prior to start)
• Ability to coordinate DME, SNF placement, and transportation REQUIRED

• Behavioral health-only experience NOT accepted
• Weekend rotation required
• RTO limited to 7 days max
