Heartline Staffing, in partnership with Purple Cow Recruiting, is seeking an experienced Travel Registered Nurse – Case Manager for an acute care hospital assignment in Mission Viejo, California.

This role focuses on inpatient care coordination and discharge planning for medically complex adult and geriatric patients in a high-volume hospital environment.

POSITION OVERVIEW
This opportunity is ideal for experienced nurse case managers who are confident managing high patient volumes, coordinating complex discharges, and working independently within a fast-paced hospital system.

ASSIGNMENT DETAILS
• Contract Length: 13 weeks
• Shift: Days, 5x8 (08:00–16:30)
• Patient Ratio: Approximately 1:25
• Patient Population: Adult and geriatric
• Setting: Acute care hospital
• EMR: Epic
• Weekend Requirement: Rotating weekends
• Floating required to Laguna facility

COMPENSATION
• Gross: $3,059 per week
• W2: $28.00 per hour
• Tax-Free Stipend: $1,939 per week

ROLE RESPONSIBILITIES
• Coordinate inpatient care and discharge planning
• Arrange DME, SNF placement, and transport services
• Collaborate with physicians, social services, and care teams
• Ensure compliance with payer and regulatory guidelines
• Manage high-volume caseloads efficiently
• Support continuity of care across settings

WORK ENVIRONMENT
• Acute care hospital setting
• High patient volume and fast-paced workflow
• Strong interdisciplinary collaboration
• Independent decision-making required

• Minimum 2 years acute care case management experience REQUIRED
• Prior travel experience REQUIRED (HARD STOP)
• Acute hospital case management experience REQUIRED
• Epic experience REQUIRED
• Must be able to independently arrange DME, SNF placement, and transport REQUIRED
• Active California RN license REQUIRED (pending accepted, must be active prior to start)
• Must be comfortable with high-volume caseloads REQUIRED
• Ability to float to Laguna facility REQUIRED
• Weekend rotation REQUIRED
• RTO limited to 7 days maximum
