Heartline Staffing, in partnership with Purple Cow Recruiting, is seeking an experienced Travel Registered Nurse for a Home Health assignment in Medford, Oregon and surrounding areas.

This role supports adult patients in a home setting, delivering skilled nursing care through independent visits and care coordination.

POSITION OVERVIEW
This is a field-based role ideal for nurses comfortable working independently, managing their own schedules, and delivering comprehensive in-home patient care.

ASSIGNMENT DETAILS
• 13-week contract
• Day shift | 5x8s (08:00–16:30)
• No weekends
• No on-call
• Up to 6 visits per day (30 productivity points standard)
• 1:1 patient care model

COMPENSATION
• Gross: $2,646 per week
• W2: $35.00 per hour
• Tax-Free Stipend: $1,246 per week

CLINICAL RESPONSIBILITIES
• Perform Start of Care, Admissions, Recertifications, and routine visits
• Provide wound care and medication management
• Complete discharge visits
• Maintain infection control standards
• Document using OASIS and EMR systems

WORK ENVIRONMENT
• Field-based home health role
• Adult patient population
• Travel required between patient homes (up to 50+ miles)
• Mileage reimbursed
• Independent workflow

• Active RN license REQUIRED (in hand prior to submission)
• Minimum 1 year Adult Skilled Home Health experience REQUIRED
• OASIS documentation experience REQUIRED (HARD STOP)
• Experience with Start of Care, Admissions, and Recertifications REQUIRED
• BLS certification REQUIRED
• Experience with wound care and medication management REQUIRED
• Computer documentation experience REQUIRED
• Reliable transportation REQUIRED
