Heartline Staffing is seeking an experienced Registered Nurse for a Home Health travel assignment in Medford, Oregon. This role supports adult patients in a home health setting and requires strong independent clinical judgment, time management, and documentation skills.

POSITION OVERVIEW
This opportunity is ideal for experienced home health nurses who are comfortable working independently, managing a full patient caseload, and completing comprehensive documentation including OASIS assessments.

ASSIGNMENT DETAILS
• Contract Length: 13 weeks
• Shift: Day shift, 5x8 (0800–1630)
• Schedule: Weekdays
• No weekends
• No on-call requirement
• Floating not required

CLINICAL RESPONSIBILITIES
• Admission assessments and start of care visits
• Routine home health visits
• Recertifications and resumption of care
• Discharge planning and visits
• Wound care and infection control
• Medication administration and management
• Pain assessment and patient education
• Accurate and timely documentation (OASIS)

WORKFLOW EXPECTATIONS
• Up to 6 patient visits per day
• Productivity expectations in line with home health standards
• Travel within assigned service area required

• Minimum 1 year adult skilled home health RN experience REQUIRED
• OASIS documentation experience REQUIRED
• Experience with admission assessments and start of care REQUIRED
• Experience with routine visits, recertifications, and discharge REQUIRED
• Wound care experience REQUIRED
• Medication administration and management REQUIRED
• Infection control experience REQUIRED
• BLS REQUIRED
• Strong independent clinical judgment REQUIRED
• Reliable personal transportation REQUIRED
