Heartline Staffing, in partnership with Purple Cow Recruiting, is seeking an experienced Travel Registered Nurse for a Home Health assignment in Medford, Oregon.

This role supports adult home health patients in a field-based setting, focusing on skilled nursing visits including start of care, routine visits, recertifications, and discharge planning.

POSITION OVERVIEW
This opportunity is ideal for independent nurses who are comfortable working in the field, managing their own schedule, and delivering high-quality care across multiple patient homes.

ASSIGNMENT DETAILS
• Schedule: Days, 5x8 (08:00–16:30)
• Contract Length: 13 weeks
• Patient Ratio: 1:1 (up to 6 visits/day; ~30 productivity points)
• No weekends
• No on-call
• No floating
• Guaranteed hours (facility may call off 1 shift per 2-week period)

COMPENSATION
• Gross: $2,646 per week
• W2: $35.00 per hour
• Tax-Free Stipend: $1,246 per week
• Mileage reimbursed at IRS standard rate

CLINICAL FOCUS
• Start of care and admission assessments
• Routine skilled nursing visits
• Recertifications and discharge planning
• Wound care and medication management
• Infection control and patient education

UNIT HIGHLIGHTS

• Epic charting with OASIS documentation

• Minimum 1 year adult skilled Home Health RN experience REQUIRED
• BLS REQUIRED
• Admission assessment experience REQUIRED
• Start of care experience REQUIRED
• Routine visit experience REQUIRED
• Recertification experience REQUIRED
• Discharge planning experience REQUIRED
• Wound care experience REQUIRED
• Medication administration and management REQUIRED
• Infection control experience REQUIRED
• Computerized documentation experience REQUIRED
• Reliable transportation REQUIRED
• OASIS experience preferred (must disclose if not present)
• RTO must be submitted at time of application
