Summary:
The Physical Therapist, (PT), performs evaluations and administer treatments prescribed for the spectrum of physical disabilities, supervises PT aides, participates in in-service and quality assurance programs and other duties and administrative tasks as directed by supervisors.
Duties and Responsibilities:
Provides physical therapy services to patients according to a written individualized plan of care. This may include, but not be limited to:
· Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the environment.
· Assesses for muscle E a goal of rehab, more.
· Directing physical therapy treatment.
· Provides equipment required to increase patient function and independence.
· Instructing patients and families/caregivers in the use and care of therapeutic appliances.
· Determining priority needs for physical therapy
· Reporting to physician patient's reaction to treatment or changes in condition.
· Initiates physical therapy program and instructs other personnel and/or family/ caregivers in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.
· May educate patient in the use of prosthetic device.
· Prepares and submits clinical and progress summaries based on the attainment of goals.
· Participates in patient discharge planning. Prepares and submits a clinical progress summary based on the attainment of goals directed by organization policy.
· Provides physical therapy consultation to families/caregivers, as appropriate.
· Provides in-service education programs for nursing personnel as needed.
· Participate in the interdisciplinary group.
· Actively participates in the quality assessment performance improvement teams and activities.
· Participate in home health orientation program.
· Provides supervision of physical therapy assistants and other aides, as appropriate.
Education, Training and Licenses Required:
· The Physical Therapist must be a graduate of a CAPTE (Commission on Accreditation and Physical Therapy Education) accredited university/college
· Current California licensure
· Current BLS certification
Experience Required:
· No prior work experience is required but it is desirable.
Knowledge/Skills/Abilities Required:
· Thorough command of accepted PT evaluation and treatment techniques.
· Good documentation, communication, organizational and problem-solving skills.
· Ability to interact effectively with patients of all ages and developmental stages.
· Must be able to communicate in English verbally and in writing.
Equipment Knowledge Required:
The PT will demonstrate competency in theory and practical use of all electrical, thermal and mechanical equipment used in this department. Ability to use office equipment with dexterity and A.V. equipment. Competency with the electronic documentation system must be obtained within 30 days of hire with full proficiency by 90 days of hire.
Safety Requirements:
Employees are responsible for all aspects of the hospital safety and health program including compliance with safety rules and regulations, remedying unsafe working conditions, and for continuously practicing safety while performing duties. Sonoma Specialty Hospital policy prohibits employees with patient contact or those who prepare products for patient use from wearing artificial nails.

While performing the duties of this job, the position is required to:
- Move within a complex hospital environment.
- Walk; stand and sit for extended or sustained periods of time.
- Occasionally stoop, kneel, crouch, and reach.
- Push, pull, lift, and carry objects.
- Exert up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, with periodic lifting and carrying of supplies or equipment up to 25 pounds.
- Grasp, hold, pick up, and reach with hands and arms.
- Express or exchange ideas accurately and quickly through continuous speech and hearing.
- Perceive the nature of sounds adequately (with no less than a 40 dB loss at 1000 Hz and 2000 Hz, with or without correction).
- Read, write by hand, and use a keyboard and mouse extensively.
- Possess visual acuity (near and far from 20 inches to 20 feet), depth perception, field of vision, ability to focus, and the ability to identify and distinguish colors, with or without correction.
- Feel sizes, shapes, temperatures, and textures by touch.
Supervisory Requirements (if applicable): none. Work Environment:
Able to adapt to sudden changes and willing to care for all types of patients regardless of age, ethnic background, religion, sexual preference, educational or economic status. Able to adapt to specific shift routine, i.e., sleep/wake pattern.
