As the HCHB Operations Systems Lead at Transitions Care will be an integral part of building and supporting the Company’s EMR workflow and systems standardization across all branches. This role will improve time to first visits as well as reduce missed visit rate. This position will also track orders with clear ownership outlining aging, escalation, closure and appropriate visibility. Reduce open documentation backlog and expedite the documentation closure process. This position will produce a weekly workflow report and branch accountability process. Reduce process workaround behavior with more consistent coaching and training.
Remote with the ability to travel 20-30% regionally
Major Responsibilities
Workflow ownership and standardization
- Own and standardize HCHB workflows across all branches
- This includes but is not limited to scheduling, ordering and documentation closure
- Establish rules and cutoffs and various escalation pathways for workflow
- Identify recurring workflow breakdowns and the associated fixes with branch owners
Adoption and Brand execution
- Create simple SOPs and pathways for all branches to follow
- Conduct branch audits and provide coaching
- Hold office hours and super user trainings
- Drive accountability through collaboration with the branch leadership until execution is consistent
Reporting and operating Cadence
- Weekly workflow health by breach
- Time to first visit, missed visits, open documents, open orders, aging orders etc.
- Weekly workflow huddle with the branch owners to assign actions and owners to actions with clear due dates
- Maintain a clear and prioritized backlog of workflow defects and improvements to ensure process improvement for the long run
Workflow system optimization
- Maintain workflow standards inside HCHB such as statuses, templates, worklists and rules
- Partner with the IT team and external vendors to improve workflows that impact changes for process improvement
- Lead UAT and change control workflow impacting all branch updates

Education & Experience:
- Bachelor’s degree in nursing, information technology or relevant field
- 3-5 years of HomeCare HomeBase experience
- Strategic and curious mindset; willing to dive into the details and give suggestions to complex issues
- Ability to identify and bring trends to the forefront; being proactive without direct oversight
- Experience in Healthcare with knowledge of Medicare and Medicaid
