Under the direction of the Clinical Manager, the Family Outreach Clinician provides direct services to children and families, allowing easy access, timely response and flexibility to deliver services that are home and community based in a family centered approach.
Sign-on bonus 50% paid upon start date and 50% paid after 3 months. Amount based on 40 hrs. full-time per week.
DIRECT CLINICAL SERVICES:
Provides assessment and counseling to children, families and/or groups with behavioral and/or emotional mental health needs. Services will be provided in the home, school or office as needed. Facilitates referrals and communication between agencies for other services on behalf of the child. Attends team meetings to plan and monitor services. Provides case management, collateral contact and social support, as needed. Will provide a shift or two a month on the centralized intake team.
CASE MANAGEMENT & SERVICE COORDINATION:
Makes home visits to support families, as needed. Facilitates communication between agencies on behalf of the child. Attends team meetings to plan and monitor services. Provides case management, and/or coordinates with the assigned case manager.
Provides consultation services to teachers , community partners, and school personnel, as needed. Consultation can be educational or evaluative. Also provides consultation and support to staff.
Initiates and maintains, in a timely and thorough manner, the wide variety of record keeping and administrative documentation required to account for services provided to consumers – including, but not limited to, consumer specific assessments, case services, progress notes, time sheets, schedules, billing reports to third party payers and assigned periodic reports. Follows all UCS record-keeping procedures and performs other duties as necessary.
Attends all UCS and community meetings necessary to promote coordination and communication.
