This contract will provide occupational medicine health services and electronic health records management for sworn staff from both the City of Phoenix (City) Fire and Police Departments as well as public safety personnel assigned to other jurisdictions throughout the region who have contracted with Phoenix Fire Department Health and Wellness Center (PFDHWC) for these services.

Location: 50 S. 12th St., Phoenix, AZ 85034.
PFDHWC Clinic hours: 7:30 a.m. to 12:00 p.m. and 12:30 p.m. to 4:00 p.m. Monday through Friday, with a 30-minute lunch period.
PRN/Per Diem Coverage: roughly 3.5 hours/week during our business hours based on candidate's availability.
Position Summary
- Assist in the implementation of strength, conditioning, fitness, injury prevention and treatment, as part of the overall wellness for PFD’s 2,000 members and 70 plus locations.
- Manage the Physical Therapy Clinic including physical therapists, physical therapy technicians, strength and conditioning coach, massage therapist and athletic trainers.
- Performs physical examination followed by development and implementation of a plan of care with full-service treatment for work-related injuries of firefighters.
- Interface and collaborate with the PFDHWC medical team and referring physicians regarding firefighters’ overall plan of care, reassessments, and work status/goals.
- Interface(s) with PFD’s Academy regarding fitness and conditioning, injury prevention.
- Manages Peer Fitness Trainer program and fitness training education, nutritional counseling, continuing education, and injury prevention.
- Coordinate fitness equipment delivery for Physical Therapy and fire stations.
- Prepares and presents educational materials to firefighters on current health trends in strength training, fitness, conditioning, flexibility, nutrition and treatment techniques.
- Participate in data collection and analysis of injuries to reduce prevalence and severity of injury within the fire service and present relevant information to the fire community.
- Utilizes PFDHWC data to identify trends and develop injury prevention strategies.
- Licensed Physical Therapist in the State of Arizona.
- Ten years of experience as a physical therapist in outpatient/sports injury assessment.
- Working knowledge of and experience with the IAFF/IAFC Wellness Fitness Initiative, NFPA 1582 and OSHA 1910 relating to firefighters.
- Current BLS certification.
This job is CONTINGENT upon contract award, client approval, completion of a favorable background investigation, and the ability to obtain and maintain a Public Trust / High Risk clearance.
