At Lifted Hospice, our Medical Director helps guide the clinical heart of our organization, ensuring that every patient receives thoughtful, appropriate, and compassionate care at the end of life.
In this role, you bring medical leadership, clinical oversight, and steady guidance to the interdisciplinary team. You help ensure that care decisions are timely, appropriate, and aligned with each patient’s goals. Your presence supports both families and clinicians, bringing clarity, confidence, and excellence to every stage of care.
Your Role in the Mission
As Medical Director, you are responsible for the medical oversight of hospice services, including physician services and the overall medical component of the organization. You ensure that all care aligns with regulatory standards, clinical best practices, and Lifted’s commitment to dignity and compassion.
You oversee and support the management of patients’ terminal illnesses and related conditions in collaboration with the attending physician, while promoting and maintaining standards of practice consistent with quality hospice care. You serve as an active member of the Interdisciplinary Team (IDT), contributing to coordinated, patient-centered care across all disciplines.
You collaborate closely with attending physicians, the Director of Clinical Services, and the interdisciplinary team to support eligibility determinations, care planning, and ongoing clinical direction. You provide clinical oversight, guidance, and education to ensure consistency, quality, and compliance across all patient care activities.
You also serve as a clinical resource within Lifted and the broader medical community, supporting physician collaboration, regulatory alignment, and the appropriate use of hospice services.
Key Responsibilities:
- Provide medical oversight – Lead and oversee the medical component of the hospice program, ensuring care aligns with regulatory standards and Lifted clinical expectations.
- Support eligibility determinations – Review and certify patient eligibility for hospice services, including admissions and recertifications, in accordance with Medicare Conditions of Participation.
- Collaborate on patient care – Participate in IDT meetings and contribute to the development and oversight of individualized plans of care.
- Provide clinical guidance – Offer consultation, direction, and education to the interdisciplinary team and attending physicians to support quality care delivery.
- Serve as attending physician when needed – Provide medical care and decision-making when the patient’s attending physician is unavailable.
- Complete face-to-face encounters – Conduct required face-to-face visits and provide clinical documentation supporting continued eligibility.
- Engage with the medical community – Maintain relationships with physicians and healthcare providers to support coordination of care and appropriate hospice utilization.
- Support quality and compliance – Participate in QAPI initiatives, audits, and ongoing performance improvement efforts to ensure regulatory compliance and clinical excellence.
- Contribute to policy development – Assist in the review and development of clinical policies, procedures, and standards of care.
- Promote education and growth – Support staff education and ongoing development related to hospice care and end-of-life clinical practices.
- Ensure compliance and integrity – Adhere to all CHAP, Medicare, Medicaid, and applicable regulatory requirements in all clinical and documentation practices.
- Remain flexible and responsive – Support clinical needs across the organization, including availability for consultations, urgent needs, and local travel as required.
Skills and Qualifications:
- Graduate of an accredited college/university with a Doctor of Medicine degree or Doctor of Osteopathic Medicine degree and is fully licensed to practice medicine.
- Currently licensed to prescribe pharmaceuticals and medical treatments in the state, with active enrollment in Medicare and Texas Medicaid (TMHP) as an authorized ordering and referring provider, or eligible to obtain enrollment upon hire.
- Knowledgeable about the psychosocial and medical aspects of hospice care.
- Ability to establish and maintain effective working relationships with all patients' families, staff, the Governing Body, the Interdisciplinary Group/Team, and other physicians.
- Reliable transportation and valid auto liability insurance.
Where You’ll Work
This role is a hybrid position, combining administrative, collaborative, and field-based responsibilities. You may work from a Lifted office, remotely, or within the community depending on the needs of the organization.
You will participate in interdisciplinary team meetings, collaborate with clinical leadership, and may conduct patient visits in homes, hospitals, or facilities as needed. This role requires flexibility, responsiveness, and the ability to engage across multiple care settings.
