Description

The Case Worker is responsible for providing casework services to families and children on assigned caseloads, helping the client get connection to other needed services, and serving as a member of the treatment team for cases assigned.  In addition, they are responsible for communicating with other team and staff members to ensure integrated service delivery to the client and serves as a primary provider of those services.


Job Responsibilities:

  • Provides indicated direct casework services to client on assigned caseloads and in accordance with local department of social services regulations.
  • Responsible for submitting written reports for the client record as required by the agency according to time frames set by the program.
  • Responsible for preparing required external accountability reports and forms on case progress for assigned cases to local departments of social services, according to time frames set by local departments of social services.
  • Participates as a member of a treatment team for each assigned case. In addition, responsible for communicating with other team and staff members to ensure integrated service delivery to the child/family.
  • Serves as a primary provider of appropriate direct services to the child’s family based on need assignment and the treatment plan for the child/family.
  • Coordinates discharge planning and provides indicated aftercare service including direct service and linkage to other supportive or needed services.
