About the Role

The Behavioral Health Lead is a working leader who provides day-to-day clinical and operational support for Social Workers and Community Health Workers while maintaining an active patient panel as a Social Worker II. This role ensures consistent delivery of behavioral health programs, supports team performance and development, and partners closely with Behavioral Health leadership to achieve quality, access, and outcome goals.


This position is ideal for a Licensed Clinical Social Worker who enjoys mentoring others, thrives in a collaborative, interdisciplinary environment, and is motivated to improve care for patients with complex medical, behavioral, and social needs.

  

Behavioral Health Lead Responsibilities

  • Provide daily oversight of behavioral health staffing, coverage, and workload allocation in partnership with the Manager, Behavioral Health
  • Support onboarding, training, coaching, and ongoing professional development of behavioral health team members
  • Serve as a clinical resource and subject matter expert, offering hands-on guidance aligned with program standards and policies
  • Monitor productivity, quality, documentation, and compliance through reporting and audits
  • Participate in quality improvement initiatives and support team members in meeting performance goals
  • Identify operational gaps or barriers and collaborate with leadership to implement solutions
  • Assist with recruitment, orientation, training, and performance feedback for team members
  • Facilitate team meetings, trainings, and coaching sessions
  • Promote a positive, collaborative work environment focused on safety, wellbeing, and professional growth
  • Represent the organization in select external meetings with community partners
  • Note: This role does not include direct HR responsibilities.

Clinical Responsibilities (Social Worker II)

  • Provide behavioral health assessments and brief, evidence-based interventions for an assigned patient panel
  • Utilize evidence-based modalities such as CBT, behavioral activation, and problem-solving therapy
  • Develop and monitor person-centered care plans using measurement-based treatment approaches
  • Collaborate with interdisciplinary care teams, including providers, psychiatrists, nurses, and care navigators
  • Support psychotropic medication management through care coordination and patient education
  • Address social drivers of health through care management, referrals, and community resource coordination
  • Develop safety plans and support patients with complex psychosocial and behavioral health needs
  • Document all patient care and coordination activities in the electronic health record
  • Deliver culturally responsive care that honors patients’ values, beliefs, and backgrounds

  

Qualifications

  • Master’s degree in Social Work
  • Current independent social work licensure in the applicable state
  • Minimum of 2 years of healthcare or behavioral health experience
  • Strong organizational, communication, and problem-solving skills
  • Ability to manage complex patient needs and multiple priorities in a fast-paced environment
  • Experience working collaboratively within interdisciplinary teams
  • Geriatric care experience preferred
  • Proficiency with Microsoft Office (Outlook, Excel, Word, PowerPoint) and electronic medical records
  • Reliable internet access and comfort using web-based tools and applications

 Base Salary/ Wage Range $100,000 to $110,000 plus annual bonus. Compensation for the role is commensurate with the candidate’s qualifications, skills, competencies, and experience and may fall outside of the range shown.
