Child Services Case Manager

Position Summary
Aroostook Home Health Services is seeking a compassionate, reliable, and motivated case manager to provide services under Section 28 – Rehabilitative and Community Support Services for Children with Cognitive Impairments and Functional Limitations. The ideal candidate will provide case management, and coordination of care to individuals and families accessing MaineCare Section 28: Rehabilitative and Community Support Services.
Key Responsibilities
- Provide individual and/or family support services under supervision of a licensed behavioral health professional
- Implement individualized treatment plans under the supervision of a licensed clinician
- Collaborate with families, schools, and multidisciplinary providers
- Complete required documentation in compliance with MaineCare Section 28 regulations
- Participate in supervision and team meetings
- Assist with intake assessments and client screenings
- Develop and implement service plans in collaboration with clinical staff
- Provide case management and coordinate community resources
- Participate in team meetings and case consultations
- Maintain professional boundaries and ethical standards at all times
- Evaluation and assessment of the social, emotional, physical, and developmental factors related to the client's overall service needs to identify problem focused goals
- Supportive casework provided for clients and their families geared towards promotion of independence in functional, living and social skills.
- Acts as intermediary between clients and other community agencies in the collaboration of care.
- Assists clients and their families to locate and utilize community resources to meet ongoing needs
- Planning and participation in staff development activities for direct care staff working as behavioral health professionals
- Prepares clinical/progress visit notes on the day of the visit and incorporates it in the clinical record weekly. Provides summaries and re-evaluations as indicated in the time frames specified.
Requirements
Qualifications
- Strong organizational and documentation skills
- Reliable transportation and ability to travel within service area
- Associates or Bachelor’s degree- preferred but not required
- 2 Years experience in Human Services Related fields
- Knowledge of community resources and referral processes
- Strong communication and documentation skills
- Ability to work independently and as part of a team
- Must be able to travel to prospective clients' residences
- Must have a car with required insurance coverage and a State driver's license
Preferred Skills
- Experience in behavioral health, mental health, or related field preferred
- Knowledge of trauma-informed care principles
- Flexible availability (after school hours preferred)
- Experience working with children with developmental disabilities, autism spectrum disorder, ADHD, trauma, or behavioral health challenges preferred
