  • Salaried, exempt
  • New hires can expect to be placed on our starting wage scale of $72,924 to $87,817 per year
  • It may be possible to earn more over time up to $102,793 per year


Work Schedule

  • Full time, M-F with Saturday rotation 3-4 times per year
  • 8a-5pm

Job Summary

Our Case Manager Supervisor is responsible for overseeing the day-to-day operations and tasks performed by our Case Managers (CM) and Community Health Workers (CHW) and ensures adequate coverage in all UCNW locations. The Case Manager Supervisor provides leadership and support to both teams.
 

Primary Responsibilities Include:

  • Provides supervision to departmental staff
  • Manages timesheets and schedules for staff to ensure adequate coverage
  • Provides support to teams and other departments as needed
  • Initiates and leads case consults and care coordinator efforts with staff
  • Supports systems work to strengthen UCNW's care coordination for patients
  • Leads patient engagement quality improvement efforts, achieves quality performance metrics and document expectations

Requirements
  • Ability to travel between UCNW locations and within service area for patient outreach.
  • Possesses a basic level of written and verbal communications skills, computational and computer skills and mathematical knowledge at a level typically acquired through completion of a general studies high school program.
  • Experience working in a community health center and familiarity with social services in Whatcom County, or equivalent relevant lived experience preferred. 
  • Experience mobilizing resources for low-income or underserved populations strongly preferred.
  • Possesses a full understanding of all aspects of the profession or trade that are used within UCNW.
  •  Minimum one (1) year of supervisory experience required.
  • Minimum one (1) year Case Manager, Community Health Worker, or social services experience required.
  • Ability to work in a fast-paced, community health center environment with frequent interruptions, patient interactions, and occasional emergency situations.
  • Ability to work outside normal clinic hours periodically, which may include weekends and evenings.
  • Excellent time management and organizational skills with the ability to meet deadlines and work under pressure with frequent interruptions.
  • Ability to understand and respond effectively and with sensitivity to a diverse population.
  • Knowledge of health insurance options including Medicare, Medicaid, private insurance, and managed care programs preferred.
  • Keyboarding speed of 30-40 wpm and data entry skills; accuracy is essential.
  •  Ability to communicate multilingually, a plus.
