Schedule: Monday – Friday: 8:30 a.m. – 5 p.m. Candidate must be available to work weekends, evenings, and holidays as needed to meet the needs of the program.

Program:
SBCS' Youth in Transition (YIT) program offers seamless high-quality services to youth transitioning out of the foster care system. Our trauma-informed services and activities provide foster youth with opportunities to enhance their self-sufficiency as they move into adulthood. YIT supports youth to address past trauma, teach independent living skills, provide transitional housing, safe places, employment/education pathways, experiences that build self-confidence, as well as identify permanent supportive adults and other resources that assist to develop the skills necessary to succeed independently.
Responsibilities:
Under the supervision of the Youth in Transition Program Director the Youth and Family Development Associate (YFDA) will provide Youth Systems Navigation (YSN) Outreach and Case Management. YFDA will be responsible for providing access to eligible youth and young adults that are experiencing homelessness or housing instability resources, housing navigation and/or assessment to support an individualized service plan that assists in obtaining shelter or housing solution. YSN is committed to providing services that are trauma informed, strength based, person center and empowerment oriented.
Duties:
- Support in the implementation and evaluation of YSN services that provide assistance to our most vulnerable youth and young adults in a trauma informed approach that adheres to and upholds the standards of South Bay Community Services.
- Outreach and canvass high-risk areas to locate and engage in street-based services with youth and young adults that are experiencing homelessness or housing instability while building a trusting rapport over time.
- Provision of basic needs, emergency referrals, housing navigation, stabilization supports, case management, and service plan that links to resources
- Facilitate referrals to career pathway, life skills, mental health, mentorship, educational and other support programs.
- Connect youth and young adults to supports that to achieve increased economic stability and self-sufficiency.
- Assist youth and young adults to create a housing plan that identifies a transition out of homelessness.
- Must be flexible to provide services in the community as needed.
- Attend all required agency trainings and meetings.
- Maintain documentation in Efforts to Outcomes (ETO), Homeless Management Information System (HMIS), Clarity, and/or other identified data bases and report on all youth and young adults in a timely manner.
- Maintain confidentiality and privacy standards of youth and young adults protected health information and other applicable information and material in accordance to agency and center policies, and applicable regulations and laws, including HIPAA
- Maintain warm, welcoming office grounds and facilities.
- Attend all agency required meetings and trainings.
- As other duties assigned.
- Bachelor’s Degree in social work, psychology, human services, sociology, or related field OR 4 years of experience working with children and adolescents including the documentation of services following a standard protocol as an equivalent. .
- Experience working with TAY youth and transitional housing programs preferred.
- Demonstrated leadership and problem-solving abilities, training skills, creativity, and public speaking.
- Ability to provide quality trauma-informed services in a fast-paced environment, specializing in crisis intervention and de-escalation, motivational interviewing, and Seeking Safety.
- Ability to (re) prioritize and multi-task is a must.
- Available to work evenings, weekends and holidays.
- Ability to work with a diverse population, possess cultural sensitivity, and maintain appropriate boundaries with clients.
Conditions of Employment
- Employment may be conditioned upon satisfactory completion of certain background clearances which may include a Department of Justice criminal background check requiring my fingerprints (Live Scan), screening through the National Sex Offender Public Website, drug test, tuberculosis (TB) test, and such additional clearances as may be required under the program.
- Candidate is required to have reliable transportation; therefore, the candidate must successfully complete a motor history check. Candidate must also possess and maintain a current valid California driver’s license and valid auto insurance policy. (
Physical Demands:
- Mentally and emotionally: Ability to work under deadlines, various projects and dealing with clients in crisis and difficult situations.
- Must be able to remain in a stationary position during shift.
- Moves throughout the building.
- Moves, raises, or lowers (includes upward pulling) an object or equipment up to 50 lbs.
- Transports an object, from 20 to 50 lbs. across the facility, for various needs.
- Ability to exert force upon an object so that the object moves away from the force.
- Ability to exert force upon an object so that the object moves toward the force (includes jerking)
- Ascending or descending stairs and ladders when applicable. Body agility is emphasized.
- Balancing: Maintaining body equilibrium to prevent falling when ascending, descending, traversing.
- Stooping: Bending body downward and forward by bending spine at the waist, requiring full use of the lower extremities and back muscles.
- Handling: Seizing, holding, grasping, turning, or otherwise working with objects or office machinery.
- Communicating: Expressing or exchanging ideas. Communicating is important for those activities in which workers must impart information to clients or to the public, and in those activities in which they must convey detailed or important instructions to other workers accurately, or quickly.
