LOCATION: Peace Baptist Church - Hamtramck, MI 48212


POSITION SUMMARY: SEMHA is seeking a compassionate and motivated Community Health Worker (CHW) to join the Wayne FQHC Program located in the city of HAMTRAMCK. The Community Health Worker is a part of an inter-disciplinary clinical care team at a health center who helps increase access to health services for people through outreach, encouraging engagement, education, and peer support.


REQUIREMENTS: 

  • Conduct patient outreach and provide education on preventive screenings and chronic disease management.
  • Address HEDIS care gaps and engage high emergency department utilizers to connect with their PCP.
  • Perform Social Determinants of Health (SDOH) screenings and link patients to appropriate community resources.
  • Coordinate referrals, assist with scheduling, and help remove barriers to care.
  • Maintain accurate documentation, outreach tracking, and monthly reporting for Health Plans.
  • Participate in community outreach events and support site visit preparation (e.g., HRSA, AAAHC).
  • Maintain HIPAA compliance and confidentiality at all times.
  • Support patient panel management and other duties as assigned.

EDUCATION & QUALIFICATIONS:

  • High school diploma required.
  • Bachelor’s degree in health sciences or related field preferred.
  • MICHW certification preferred.
  • Experience in community work, education, or healthcare strongly preferred.
  • Strong communication, organizational, and interpersonal skills.
  • Ability to multitask in a fast-paced environment and advocate for patients.
  • Proficiency in Microsoft Office preferred.
  • Valid driver’s license and reliable transportation required.

Salary: Starting at $19.00 hourly
