Behavioral Health Care Manager
Description

I. BASIC FUNCTION:


Responsible for providing care management for patients of FCHC, supervising BH care managers, and assist in 

coordinating the BH department.

III. PRIMARY RESPONSIBILITIES:


1. Function as the lead care manager within the integrated primary care and behavioral health team, supervise other 

care managers. 

2. In coordination with the BHC and primary care provider, enroll eligible and interested patients into the Circle of 

Care project. 

3. Once the patient is enrolled, track patient progress towards grant outcomes. 

4. As often as is clinically appropriate, face-to-face visits and telephone calls with Circle of Care patients and other 

members of the patient’s network over the course of their enrollment in the project (3-12 months). 

5. Establish and maintain a patient registry to monitor clinical outcomes as well as key processes of care. 

6. Arrange for inpatient, partial hospitalization, intensive outpatient programs as needed. Assist with discharge 

coordination. 

7. Coordinate information flow between the PCP, BH providers, CHW, and other FCHC staff within the Circle of Care 

program. 

8. Lead care team meetings and document clinical information/decision-making in the patient registry. 

9. Identify barriers to treatment and possible solutions, including coordinating/offering transportation when needed. 

10. Establish and maintain cooperative referral relationships with community agencies, organizations, and 

professionals. Together with the other care managers, maintain an up-to-date referral listing. 

11. Facilitate referrals to other outpatient therapists as indicated. 

12. Facilitate referrals to psychiatry as needed. 

13. Participate in staff in-service programs related to social services and/or behavioral health.

15. Supports the CBHO in the following:


a. Prepare the monthly BH expense report for the CFO, including grant and non-grant related expenses.

b. Prepare and monitor BH departmental annual budget. 

c. Prepare grant budgets and manage grant reporting of expenses and outcomes

d. Prepare and monitor annual BH encounter budget

e. Prepare the annual BH CQI report

f. Prepare and submit monthly BH reports for the FCHC Board of Directors

g. Prepare Quality Improvement reports for discussion at the BH departmental meetings

16. Facilitate monthly Trauma Informed Care meetings

17. Attend Quality Champion Team meetings; assist CBHO in advising the primary care team regarding BH screening

18. Coordinate BH peer reviews, annual evaluations, and QI projects

19. Coordinate the agenda and keep minutes for all BH meetings.

20. Prepare content/updates/announcements for all FCHC team meetings (CAR, FPSE, provider, all staff)

21. To lead the care managers

a. Create and update job descriptions as needed

b. Recruit, hire, and onboard BH Care Managers

c. Determine encounter targets

d. Regular meetings to assure quality work

e. Yearly performance appraisals

IV. PERIODIC DUTIES:


1. Contributes services to community public health activities as appropriate.

2. Participates in Center work team as needed.

3. Participates in Center staff problem solving groups.

4. Performs other duties as assigned.

5. Attend monthly BH manager meetings and CQI meetings.

V. WORKING RELATIONSHIPS:

Inside Center: All Inclusive.

Outside Center: Patients, community organizations and agencies, professional colleagues and organizations, 

and institutions of learning.


VI. QUALIFICATIONS:


1. Masters in Social Work AND Licensed Clinic Social Work (LCSW) required.

2. Minimum of three (3) years experience working in the field of social work or behavioral health care is required.

3. Work experience or strong desire to work in urban community health setting preferred.


VIII. CONTINUING EDUCATION:


1. Active professional development as demonstrated through attendance of seminars, continuing education 

programs, etc. is strongly recommended. 

2. LCSW: Satisfactory maintenance of continuing education standards as established by the Division of Professional 

Registration of Missouri is required.
