  • Provide clinical oversight and leadership to home health staff, ensuring high standards of care and adherence to agency policies and regulatory requirements.
  • Oversee daily clinical operations of home health services to ensure compliance with federal, state, and local regulations, including Medicare and Medicaid guidelines.
  • Supervise, mentor, evaluate, and support nursing and clinical team members to promote professional growth, collaboration, and high performance.
  • Review and monitor clinical documentation and electronic health records (EHR) to ensure accuracy, completeness, and regulatory compliance.
  • Develop, implement, and maintain clinical protocols, policies, and procedures to enhance patient care quality, safety, and consistency.
  • Collaborate with interdisciplinary team members to coordinate patient care plans and optimize patient outcomes.
  • Monitor clinical performance metrics, identify improvement opportunities, and lead quality assurance and performance improvement initiatives.
  • Assist with staff education and training to support continued professional development and adherence to best practices.
  • Serve as a liaison between staff, patients, families, physicians, and external agencies to facilitate effective communication and service delivery.
  • Participate in strategic planning, operational decision-making, and agency initiatives to support organizational goals.
  • Manage multiple priorities with strong organizational skills, effective leadership, and sound clinical judgment.

Requirements

Current state license as one of the following: 

  • Physician
  • Physical Therapist
  • Speech-Language Pathologist
  • Occupational Therapist
  • Audiologist
  • Social Worker
  • Registered Nurse
  • Active professional license to practice in the State of Iowa.
  • 3–5 years of experience in home health or hospice care, including prior supervisory or management experience.
  • Work Schedule: M-F 8AM-5PM and includes participation is services on call rotation. This is not a remote position, attendance in office is required
  • Strong knowledge of home health regulations, standards of care, and best clinical practices.
  • Experience or training in home health reimbursement for Medicare and Medicaid preferred.
  • Proficiency with Electronic Health Records (EHR) and clinical documentation processes.
  • Demonstrated leadership, decision-making abilities, and effective interpersonal communication skills.
  • Commitment to professional ethics, high standards of practice, and adherence to agency policies.
  • Reliable transportation, valid driver’s license, and adequate auto insurance as required by agency personnel policies.
  • All offers of employment are conditioned upon the result of a criminal background check conducted by the Iowa Division of Criminal Investigation, as required by law and a negative finding on the Office of Inspector General’s Excluded Provider List.
