GENERAL DESCRIPTION
The HIV/AIDS Care Coordinator is responsible for providing Ryan White Case Management services to eligible clients under the Ryan White HIV/AIDS Program (RWHAP). Case Management includes Medical Case Management, such as client-centered activities focused on improving health outcomes and treatment adherence and non-medical case management, such as guidance and assistance in accessing medical, social, community, legal, financial, and other needed services. Brief Contact Management and Graduated Client Outreach Services, as applicable. In addition to Ryan White Case management, this position may also assist in case management of other HIV and STI patients.
ESSENTIAL DUTIES
- Conduct comprehensive client assessments to identify medical, psychosocial, and service needs and develop individualized care plans.
- Coordinate and facilitate access to services, ensuring timely linkage to medical care, supportive services, and community resources.
- Monitor and re-evaluate care plans at least every six months, adjusting services as needed to support treatment adherence and client goals.
- Provide benefits counseling and advocacy, assisting clients with Medicaid, Medicare, pharmaceutical programs, insurance marketplaces, and other funding resources.
- Support treatment adherence and client engagement, maintaining open communication between clients and their clinical care teams.
- Collaborate with internal and external partners, including CHC programs, outside organizations, and community agencies to coordinate comprehensive care.
- Support program operations, including Peer Support Group services, KS-CAC participation, and attendance at staff meetings, KDHE meetings, conference calls, and quality improvement activities.
- Collect, document, and report required program data, including RWHAP reporting, infectious disease data tracking, and completion of daily Time & Effort reports.
- Maintain compliance with program standards, ensuring accurate documentation, adherence to regulatory requirements, and protection of patient confidentiality in accordance with HIPAA and organizational policies.
- Travel between clinic sites and within the community as needed to support client appointments, transportation coordination, and delivery of medications or resources.

QUALIFICATIONS AND SKILLS
- Bachelor’s Degree in Social Work, Psychology, or other Human Services required.
- Associates Degree in Social Work, Psychology, or other Human Services related degree with 3 years of case management experience may be used in replacement of Bachelor’s Degree.
- Basic Life Support certification upon hire or within ninety (90) days from the date of hire.
- Continuing education and certifications as required by CHC’s HIV/STI program.
- Must possess a valid driver’s license, as travel is required. Must maintain driving standards- no more than 2 minor or 1 major driving violations within a 3-year lookback period.

- Must be organized and able to manage/prioritize tasks to meet deadlines, adapt to change positively, able to work as a part of a team.

- Ability to handle emergency situations calmly and effectively.
- Must be computer literate, especially with Microsoft Office products, and be proficient with the Electronic Health Record software.
- Must be able to maintain good interpersonal relationships with co-workers and other members of the health care team and the organization.
- Provide customer service in accordance with the organization’s mission.
- Be courteous and respectful when interacting with patients and family members.
- Maintain patient confidentiality in accordance with the organization’s policy and procedure and HIPAA requirements.
WORKING CONDITIONS
While performing the duties of this job, the employee is regularly required to stand; use hands to finger, handle, or feel; reach with hands and arms and talk or hear. The employee is frequently required to walk. The employee is occasionally required to sit. The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Computer work is frequent. May be exposed to patients/public with communicable diseases. Travel is required for this position.
