What You Will Do:
The successful candidate will be willing and able to:
- Provide same-day warm handoffs from the medical and referral teams; deliver brief, evidence-based interventions, psychoeducation, assess risk, and provide problem-solving support within the PCBH model.
- Collaborate with PCPs, medical assistants, and BH team members; participate in case consultations; communicate patient updates and treatment recommendations to the care team. Place appropriate referrals for internal and community services.
- Provide de-escalation, safety planning, and crisis intervention within the scope of training and licensure.
- Initiate and maintain timely documentation of patient encounters, sessions, WHO requests, & case consultations.
- Manage BH inbox/voicemail, receive incoming BH calls, schedule BH appointments; conduct outreach for follow-up care and welfare checks; support patients connecting with higher levels of care.
- Conduct behavioral health screenings, assess patient needs, provide ongoing brief services under supervision, per BBS and organizational policy. Deliver brief, evidence-based interventions, psychoeducation, and problem-solving support within the PCBH model
- Participate in BH team meetings and quality improvement activities.
- Ensure adherence to assessment protocols, HIPAA, privacy, consent, and ethical standards.
- Support patient engagement, retention, and culturally responsive care.

Eligibility: Benefits are available to employees working 30+ hours per week, effective the first of the month following 30 days of employment.
Compensation Disclosure: $37.50 - $41.00 per hr.
The compensation range listed reflects Elica’s good-faith estimate of the anticipated hiring range for this position in accordance with California pay transparency requirements. Final compensation will be determined based on job-related factors, including relevant experience, education, certifications, skills, and qualification.
Language Skills (Preferred):
Bilingual or multilingual communication skills in English and Russian, Farsi, Pashto/Pushto, or Spanish are preferred to support the needs of the communities we serve, where more than 50% of patients report a primary language other than English.
- Active ASW registration in good standing with the California Board of Behavioral Sciences.
- Master of Social Work (MSW) from an accredited program.
- Experience providing brief, evidence-based interventions in clinical or community settings (e.g. at internship).
- Strong oral, written, and electronic communication skills.
- Demonstrated ability to work effectively with diverse populations and within multidisciplinary teams.
- Knowledge of HIPAA, privacy laws, and the NASW Code of Ethics.
- Experience in behavioral health integration or medical settings preferred.
- Bilingual proficiency in English and Russian, Farsi, Pashto/Pushto, or Spanish preferred
Additional Requirements
- Must have a current and valid California driver’s license and the ability to provide proof of personal auto insurance on the vehicle driven during working hours.
