Are you passionate about helping individuals and families navigate complex systems of care? We are seeking a dedicated and organized Patient Care Coordinator to join our inpatient care team. In this vital role, you will serve as a central connection point between patients, families, treatment teams, and community resources, ensuring coordinated, compassionate care throughout the patient journey.
Key Responsibilities:
- Manage a caseload of inpatient and/or residential clients
- Collaborate with interdisciplinary teams to develop, implement, and update treatment and crisis plans
- Act as a liaison between patients, families, schools, healthcare facilities, and community agencies
- Help families access mental health, educational, and community-based services
- Monitor delivery of care, track progress, and support discharge and aftercare planning
- Ensure all documentation, billing, and regulatory requirements are met
- Identify and address cultural, social, and systemic needs or barriers to care
- Participate in treatment meetings, rounds, and care coordination activities

Requirements
Required Qualifications:
- Bachelor’s degree in a human services field
- At least 1 year of full-time experience working with individuals with mental illness
Preferred Qualifications:
- 2+ years of experience in case management or healthcare/social services
- Experience in an inpatient or residential care setting
- Familiarity with Medicaid, IEPs, mental health systems, and community resource navigation
