Are you passionate about helping individuals and families navigate complex systems of care? We are seeking a dedicated and organized Patient Care Coordinator to join our inpatient care team. In this vital role, you will serve as a central connection point between patients, families, treatment teams, and community resources, ensuring coordinated, compassionate care throughout the patient journey.


Key Responsibilities:

  • Manage a caseload of inpatient and/or residential clients
  • Collaborate with interdisciplinary teams to develop, implement, and update treatment and crisis plans
  • Act as a liaison between patients, families, schools, healthcare facilities, and community agencies
  • Help families access mental health, educational, and community-based services
  • Monitor delivery of care, track progress, and support discharge and aftercare planning
  • Ensure all documentation, billing, and regulatory requirements are met
  • Identify and address cultural, social, and systemic needs or barriers to care
  • Participate in treatment meetings, rounds, and care coordination activities

Requirements

Required Qualifications:

  • Bachelor’s degree in a human services field
  • At least 1 year of full-time experience working with individuals with mental illness

Preferred Qualifications:

  • 2+ years of experience in case management or healthcare/social services
  • Experience in an inpatient or residential care setting
  • Familiarity with Medicaid, IEPs, mental health systems, and community resource navigation
