Requirements
Education
- Bachelor’s Degree Required, Field of Study: Social Work
- Master’s Degree Preferred, Field of Study: Social Work
Experience
- Minimum of 2 years’ experience in medical social work, case management or related patient support roles: Preferred
- Understanding of healthcare systems, community resources, public benefits, and social service programs
- Familiarity with interdisciplinary team collaboration and care coordination in hospital and outpatient settings
Computer Skills
- Proficient with Microsoft Office and electronic medical record (EMR) systems
Certifications & Licenses
- Active Wisconsin Social Worker license in good standing; required
- Iowa Social Worker license preferred
- Basic Life Support (BLS) certification required
