Position Summary: The Remote Clinical Manager, reporting to the Agency Administrator or designee, assists in maintaining, fostering high quality of patient care in accordance with the Agency’s objectives and policies, through planning, coordination, and implementation and evaluation of the home health services provided. This position is also responsible for education, coordination and collaboration with agency leaders regarding home health regulations and quality patient care. 

Key Responsibilities: 

1. Promote /exemplify Company mission, vision and values at all times. 

  1. Coordination and collaboration with      agency leaders on Oasis, POC, QA and liaison with Coding/Oasis      outsourcing company. 
  2. Educates and collaborates with      field clinicians, clinical managers, DOC's, Administrators on best      practices. 
  3. Participate in the development of field clinicians that routinely perform OASIS      assessments and of newly hired clinicians that will be responsible      for OASIS assessments. 
  4. OASIS and contents of POCs will be reviewed in detail for OASIS accuracy; identified OASIS corrections are to be documented      and returned to the clinician for review and concurrence. 
  5. Focus on best practice OASIS items and ensure documentation is present to support      OASIS responses. 
  6. Review to ensure appropriate utilization of resources to meet patient needs. 
  7. Ensure that the frequency listed      for each discipline is scheduled as ordered and that procedures      are scheduled as referenced on the POC 
  8. Reviews, monitors all Wellsky reports      related to QA daily workflow to ensure timely processing of MD      orders, face-to-faces, clinical documentation. 
  9. Reviews and provides necessary      education on all clinical documentation reviewed in QA Manager 
  10. Reviews daily reports and processes      to ensure timely billing/unbilled. 
  11. Assist with end of episode      billing to ensure unbilled meets company goals. 
  12. Assist with care coordination      within branch locations to ensure appropriate visit utilization,      episode scheduling and overall quality care 
  13. Reconciles monthly QAPI reports to      tally key metrics and share with inter-disciplinary team 
  14. Collaborates with key leadership to      ensure STAR ratings, HHCAPS and VBP exceeds state/national      levels and meets company goals 
  15. Assist with predictive key      metrics in identifying high-risk patients and/or opportunities      to provide high quality care 
  16. Supports branch location when      needed and functions as in-office Clinical Manager 
  17. Assist with ongoing QAPI      efforts, clinical chart audits as needed, utilization of SHP to      pull necessary metrics to support branch locations. 
  18. Identifies training needs in      branch locations, provides education as needed. 
  19. Review and investigate the clinical      content, diagnoses, medical history, medications, fee status and      other procedures from the records of clients. 
  20. Direct staff toward      the appropriate use of community resources. 
  21. Assist leaders with appraising      staff performance during their probationary period and annually with the      Director of Clinical Services, provides the teaching and counseling      necessary to enhance the delivery of services and the professional      development of nursing and health aide staff. 
  22. Assist with identifying and      evaluating client needs and appropriateness of home care by reviewing      the initial intake screening form before assignment of direct      care staff.? 
  23. Assist with the orientation of      new professional staff to familiarize them with Agency policies and      procedures.? 
  24. Perform other duties as directed by      Sr. Director of Clinical Services/VP HH Operations and/or Regional      Director of Operations.?? 
  25. Will be required to      travel as needed to support local offices in CM coverage, initial and      ongoing training of key staff. 

Experience: 

  • Minimum 3-5 years of experience as      a registered nurse in a Home Health Agency. 
  • 2 years of management      experience preferred.

Skills: 

  • Possess knowledge of Medicare and Medicaid guidelines essential to Home Health Coverage 
  • Possess good organizational and time management skills. 
  • Possess good problem-solving skills. 
  • Possess strong verbal/written communication and interpersonal skills. 
  • Aptitude for thoroughness and accuracy 
  • Initiative and the ability to work independently. 
  • Flexible, dependable,      and self-directed 
  • Computer, internet access, and email capabilities from home or other off-site location 

Education: 

  • Graduate of an accredited Diploma,      Associate or Baccalaureate School of Nursing.??? 

Licensure/Certification: 

  • Current driver’s      license in good standing. It is the responsibility of the      employee to renew their driver’s license before it      expires to continue employment.? 
  • Current State professional license      as an RN in any/all applicable states.? 
  • Current CPR? 

Physical Requirements: 

  • Prolonged      sitting required in an office environment which involves minimal      and occasional lifting of medical supplies and charts. 
  • If visiting patients, prolonged      standing and walking required, with the ability to bend, stoop, reach and      move client weight up to 250 pounds as well as carry up to 30      pounds. 
  • The ability to access clients’      homes which may not be routinely wheelchair accessible      is required. 
  • Hearing, eyesight and      physical dexterity must be sufficient to perform a physical assessment of      the client’s condition and to perform client care. 
  • Ability to always handle stressful      situations in a calm and courteous manner. 
  • Requires working under some      stressful conditions to meet deadlines and Agency needs. 

Environmental/Working Conditions: 

  • Ability to work flexible      schedule. 
  • Perform duties in an office      environment during Agency operating hours. 
  • Ability to travel in states      that company operates in. 
  • Some exposure to unpleasant      weather. 
  • If required to make patient visits,      may work in patients’ home in      various conditions, possible exposure to blood and bodily      fluids and infectious diseases.
