Position Summary

The Community Health Worker (CHW) – REACH Assistance is a non-exempt, temporary part-time position that supports United Way Fresno and Madera Counties’ (UWFM) efforts to connect eligible residents with utility assistance through the PG&E REACH Program administered by Dollar Energy. The CHW performs community-based outreach, client engagement, and application assistance to help households access available utility assistance resources.

This position focuses on identifying and assisting eligible individuals with completing and submitting applications through the program portal, ensuring all required documentation is collected and submitted accurately. The CHW works directly with community members to provide guidance throughout the application process, conduct follow-ups when additional documentation is needed, and help ensure successful application approvals.

This position is funded through grant resources and will remain active as long as program funding is available. The CHW is expected to work collaboratively with UWFM staff and community partners while maintaining accurate records, meeting application submission targets, and supporting outreach and program operations as needed.

Essential Job Functions

1. Outreach and recruitment

a. Conduct outreach within Fresno and Madera Counties to identify households that may qualify for the PG&E REACH Assistance Program administered through Dollar Energy.

b. Educate community members about program eligibility requirements, application processes, and required documentation.

c. Build relationships with community partners and referral organizations to identify eligible residents.

d. Develop and monitor key performance indicators (KPIs) related to finance, administration, risk management, and organizational effectiveness. 

e. Participate in community outreach activities including resource fairs, application clinics, and community events.

2. Application Assistance and Processing

a. Assist clients in completing utility assistance applications.

b. Review applications and documentation to ensure completeness and accuracy prior to submission.

c. Submit completed applications through the Dollar Energy program portal.

d. Maintain a monthly quota for submitted and approved applications.

e. Track application status and follow up with clients when additional documentation or information is needed.

f. Schedule appointments with clients who require additional support.

3. Client Screening and Referrals 

a. Screen applicants to determine if they may qualify for additional services offered by United Way Fresno and Madera Counties.

b. Refer eligible individuals to care coordination or other support programs when appropriate.

c. Provide navigation assistance for individuals seeking services related to food, housing, transportation, employment, or other community resources.

4. Documentation and Reporting 

a. Maintain accurate records of client interactions, applications submitted, and outcomes.

b. Track application progress and program metrics to support reporting requirements.

c. Follow internal procedures for documentation and data entry.

1. Education: High school diploma or equivalent experience required.

2. Community Health Worker certification preferred or willingness to obtain certification within 18 months of hire.

3. Experience: 

a. At least 1 year of experience in community outreach, social services, customer service, or related field preferred.

b. Experience assisting individuals with applications, eligibility screening, or benefits enrollment is highly desirable.

c.  Familiarity with Fresno and/or Madera County communities preferred. 

4. Other Requirements:

a. Strong interpersonal and communication skills.

b. Basic computer skills and ability to learn program portals and internal tracking systems.

c. Bilingual or multilingual skills required.

d. Valid California Driver’s License and reliable transportation.

Physical Requirements & Work Environment

While performing the duties of this job, the employee is regularly required to move or remain in a stationary position, to perform clerical duties in the office, and operate computer and related office machinery. The employee must regularly lift and/or move up to 25 pounds of tools or other equipment. Constantly operate a variety of functions on the computer (e.g., including reading and responding to email) along with other office productivity machinery, such as telephone, calculator, print and copy machine, etc. The person in this position is required to frequently and effectively communicate with callers and community members, in person, via email and over the phone. Must be able to exchange accurate information in these situations, and able to read from a computer screen and paper documents.  

Ability to drive for extended period of time to attend meetings, public speaking events, conferences, and other periodic evening or weekend activities. Ability to conduct meetings both in-person and remotely, and ability to set up events/meetings including lifting materials and supplies such as chairs, easels, tables.   

This position typically functions indoors with a moderate noise associated with business office equipment. The position involves travel for trainings, conferences and meetings within Fresno and Madera Counties.
