Position Overview:
Alzheimer’s Los Angeles is currently seeking a Client Services Coordinator for our Enhanced Care Management (ECM) program. ECM is a benefit offered to Medi-Cal members through the managed care plans available in the county of Los Angeles. The Coordinator will be the initial contact with potential clients, explaining the program and determining eligibility. In addition, the Coordinator will help enrolled clients with making appointments, arranging transportation, and connecting with resources. Other responsibilities include assisting our Triage Manager with managing the flow of referrals. Experience required with outreach, healthcare, and entering data into computer systems. Must be fluent in both English and Spanish, with a warm and friendly demeanor.
This is a full-time, non-exempt (hourly) hybrid position requiring some in-person work in our offices or in the community in/around Los Angeles County. Occasional weekend and evening work may be required.

Responsibilities
- Complete outreach calls to eligible clients.
- Share information about ALZ LA’s ECM program in a way that helps potential clients understand the benefit of enrolling.
- Conduct outreach via phone, in-person or virtually and offer in-service presentations in spaces where potential clients live and seek care.
- Foster connections with families and potential clients and energetically/warmly promote engagement into ALZ LA ECM program by effectively building trusting relationships.
- Assist with coordinating care by helping to schedule medical appointments, assisting with transportation, and connecting clients with medical and non-medical community supports and resources.
- Maintains accurate and detailed records of all outreach efforts, contacts, meetings, and program delivery.
- Receives, reviews, and completes triage of referrals to appropriate staff within a specified timeframe and provides follow-up as needed.
Qualifications, Skills, and Experience Required
- 2-3 years’ experience in outreach and community field work, preferably in the healthcare field
- Bilingual in English and Spanish required
- Strong interpersonal and engagement skills, with the ability to build rapport over the phone and in person
- Good knowledge of Medi-care and Medi-cal, must be able to describe benefits in a clear and concise way
- Community Health Worker experience preferred
- Knowledge of Los Angeles County aging community resources, networks, and program
- Experience with navigating access to healthcare services
- Friendly, enthusiastic and positive communicator, committed to member engagement and retention
- Computer literacy with PowerPoint, Microsoft Word, and Excel
- High school diploma or equivalent
