This on-site position is located in Wilmington, Delaware. Catholic Charities of the Diocese of Wilmington is seeking a Lifestyle Coach who will be responsible for facilitating the National Diabetes Prevention Program (NDPP) on behalf of Catholic Charities in coordination and consultation with Trinity Healthcare/St. Francis Hospital. The National Diabetes Prevention Program lifestyle change program is delivered in an effective, meaningful, and compelling way via virtual learning or in-person.
The schedule for this part-time position may vary to include three weekdays on-site and requires schedule flexibility to participate in program meetings during standard business hours and host group meetings during evening/weekend hours.
Essential Functions:
- Recruits program participants by conducting targeted community outreach; receives referrals from external agencies; manages participant intake and enrollment processes.
- Leads program participants through the yearlong lifestyle change program, helping participants develop and maintain the skills needed to adopt healthy eating and physical activity habits.
- Reinforces and integrates session material by continuously building on key curriculum concepts.
- Collects and records participant data weekly for submission to the Diabetes Prevention Recognition Program (DPRP).
- Supports participants by providing information, encouraging progress, and working with the cohort to address challenges or barriers.
- Prepares each session by reviewing lesson plans and content; provides feedback to participants.
- Maintains accessibility to participants before and after sessions; follows-up with participants outside of class if they are unable to attend a class.
- Completes and sends reports, documentation and other required information to supervisor prior to program deadlines.
- Cultivates participant commitment and supports retention of participants.
- Creates a safe, judgment-free, and encouraging environment that is friendly and non-competitive for participants.
- Participates in all required Lifestyle Coach team discussions (meetings, emails, or conference calls) and professional development opportunities, offering ideas and tools for facilitation to other team members.
Minimum Qualifications:
- High School diploma and two years of experience in health, education, nutrition, exercise science, or another health-related field; or equivalent combination of education and experience.
- Excellent written and verbal communication with strong organizational and time-management skills
- Computer proficiency including Microsoft Office/365, email applications, and video conferencing software (i.e. Zoom, Teams).
- Ability to effectively deliver program content to a variety of learners via both in-person and virtual learning methods.
- Achievement of HIPAA certification and Diabetes Prevention Program Lifestyle Coach Certification, including prerequisites, within 90 days of hire.
- The ability to safeguard access of confidential or sensitive information or data.
- The ability to build trust and rapport with individuals from diverse cultural, socio-economic, and ethnic backgrounds.
- Willingness to consent to a satisfactory criminal background check and drug screening in accordance with applicable laws.
